Gallstone Ileus: Dilemma in the Management

Abstract: Gallstone ileus is a mechanical intestinal obstruction caused due to impaction of a large gallstone within the bowel. The ideal treatment of gallstone ileus remains controversial, with the main dilemma being between a one-stage and a two-stage surgical procedure. A 69-year old male patient presented with gallstone ileus. A one-stage procedure with enterolithotomy and primary closure of duodenal fistula was done. His immediate postoperative recovery was uneventful, but after 3 weeks of surgery, he developed res… Show more

“…Available options are as follows: (i) Enterolithotomy alone, (ii) two-stage surgery (enterolithotomy followed by cholecystectomy), and (iii) one-stage surgery (enterolithotomy, cholecystectomy, and fistula repair). [15] Bowel resection is necessary if perforation occurs. [1] Factors influencing therapeutic option are age, medical comorbidities, and duration of bowel obstruction.…”

“…These two cases are the first gallstone cases reported at Cipto Mangunkusumo General Hospital as the tertiary academic hospital at Jakarta, Indonesia in the last 20 years; in accordance with the literature reporting that gallstone ileus referred to a rare case. [1][2][3][4] Both cases were in young adults although the incidence of gallstone ileus in female is 3-5 times to male. 5 Gallstone ileus more commonly found at 65 years rather than under 50.…”

“…The incidence is 1-4% of all ileus cases. [1][2][3][4] It presents as mechanical bowel obstruction caused by a gallstone, which passes through the bileenteric fistula and is trapped in the digestive tract between the stomach and rectum. Ileus presents because the gallstone obstructs the Bauhini valve.…”

“…Cholecystectomy may also reduce the incidence of cholecystitis, cholangitis and carcinoma of the gall bladder 5. The cholecystectomy may be performed in a ‘one-stage surgery’, where the enterolithotomy and cholecystectomy are performed in same procedure or may be performed in a later séance, typically 4–8 weeks after the gallstone ileus, called ‘two-stage surgery’.…”
