Gait speed as a measure of functional status in COPD patients

Abstract: Chronic obstructive pulmonary disease (COPD) is a disease associated with dyspnea, fatigue, and exercise intolerance. The degree of functional debility and level of exercise capacity greatly influences quality of life and mortality in patients with COPD, and the measures of exercise capacity are to be an integral part of patient assessment but often not feasible in routine daily practice, resulting in likely suboptimal care. There is a need for simple ways to identify functional decline in the clinical setting… Show more

“…[8] Instead, there is currently the tendency to identify tests and measures that are faster, and need less space, but still reliably measure exercise capacity. [9,10] In this sense, the 4-m gait-speed (4MGS) test, or 5-repetition sit-to-stand (5STS) test are plausible substitutes for the 6MWT.…”

“…These findings are consistent with prior study results. 21 , 25 The non-significant relationship between 6MWT and the balance component of the SPPB may be explained by the fact that successful balance maintenance is a complex skill that also relies on other determinants, such as the integration and coordination of musculoskeletal systems (ie, biomechanics, range of motion, and flexibility) and neural systems (ie, motor, sensory, and higher-level premotor processes). 41 , 43…”

“…While the sit-to-stand test clearly requires quadriceps strength, 21 our data suggest that walking 4 m is more closely related to exercise tolerance than to muscle strength properties, which may explain why it appears that the 4MGS would make a better alternative to 6MWT. 25 …”
