Background: There has remained a need to better understanding of prognostic factors that affect the survival or risk in patients with human immunodeficiency virus (HIV)/acquired immunodeficiency syndrome (AIDS), particularly in developing countries. The aim of the present study aimed to identify the prognostic factors influencing AIDS progression in HIV positive patients in Hamadan province of Iran, using random survival forest in the presence of competing risks (death from causes not related to AIDS). This method considers all interactions between variables and their nonlinear effects. Methods: A data set of 585 HIV-infected patients extracted from 1997 to 2011 was utilized. The effect of several prognostic factors on cumulative incidence function (probability) of AIDS progression and death were investigated. Results: The used model indicated that using antiretroviral therapy tuberculosis co-infection are two top most important variables in predicting cumulative incidence function for AIDS progression in the presence of competing risks, respectively. The patients with tuberculosis had much higher predicted cumulative incidence probability. Predicted cumulative incidence probability of AIDS progression was also higher for mother to child mode of HIV transmission. Moreover, transmission type and gender were two top most important variables for the competing event. Men and those patients with IDUS transmission mode had higher predicted risk compared to others. Conclusions: Considering nonlinear effects and interaction between variables, confection with tuberculosis was the most important variable in prediction of cumulative incidence probability of AIDS progression.

Healthcare is one of the most important concerns of society, being extremely relevant the accuracy and quality of their services. Basically, eHealth can be considered as the area where electronic processes and communications are used to improve the quality of medical assistance. Despite the relative maturity of the field, recalls and problems related to medical devices, applications and services are still very frequent. Therefore, it is necessary to improve and expand current research to provide advances that can be transmitted to the society. As an initial step, it is essential to have a good understanding of the current state-of-the-art of eHealth. The main goal of this paper is to identify the most relevant and recent work on eHealth but, due to the immensity of the field and the scope of the journal, we will concentrate on the use of two specific technologies: databases and collective intelligence. In addition to review the main concepts related to eHealth and the most influent academic papers, we will describe projects which are essential for the correct performance of many healthcare services. As a result of our study, we reached some interesting conclusions that might be useful for future projects and we encourage to apply them in order to avoid some of the problems that we have found in existing projects.

Objectives:The subject of survival analysis mainly concentrated on the behavior of the survival times of specific organisms or objects, which is the time at which a specified event occurs from an initial epoch. The problem of censoring adds complexity to survival analysis, and specific types of tools have been developed to deal with censored observations, some of which are nonparametric and semi-parametric approaches. When the proportion of censored observations becomes higher, it is natural that the results obtained from any type of survival analysis are less reliable. In this article, an attempt has been made to meaningfully incorporate the censored observations in survival data using fuzzy theory. Method: Taking Kaplan-Meier's procedure as a base, the fuzzy numbers are used for censored observations. Findings: In the presence of censored observations, ascertaining the median survival time is always obscure; hence, fuzzy methodology has been adopted in this article. The corresponding survival curves are drawn, and the median survival time is estimated with reference to an example. Novelty: Incorporating fuzzy theory into Kaplan-Meier's procedure, particularly regarding censoring, is a novel methodology discussed here. So, the presence of impreciseness in statistical data with particular reference to survival analysis has not been considered much in the literature. The present work has addressed this issue and proposes a new type of Kaplan-Meier estimator for addressing censoring.
