Future Perspective: Immunomodulatory Therapy for Autoimmune Hepatitis

Abstract: In the last two decades, more and more light has been shed on the immunologic pathogenesis of autoimmune liver diseases, notably autoimmune hepatitis (AIH). An immunologic dysbalance with proinflammatory immune responses dominating over hepatic tolerance seems to be part of AIH pathogenesis. In detail, an impairment of regulatory T cells (Treg) is suspected. If this holds true and reduced Treg numbers or their function are pathogenic for AIH, this offers the option of cellular or immunomodulatory therapy. Howe… Show more

“…There are controversies in scientific research regarding the number of Treg present in AHI. Though the modulating function of the cell appears to be compromised [80,81]. However, in treated patients with IL-2, the Treg number is higher than untreated patients.…”

“…However, recent reports found no impairment in the numbers of Tregs in the peripheral blood of AIH patients. Instead, the intrahepatic frequency of these cells was higher and correlated with the degree of the inflammation in the liver of these patients [ 82 , 83 , 101 ]. It was argued that the discrepancies may be due to immunotherapy-induced reductions in Treg numbers and changes in Treg phenotypic markers as a result of disease activity and/or treatment [ 101 , 102 ].…”

It was argued that the discrepancies may be due to immunotherapy-induced reductions in Treg numbers and changes in Treg phenotypic markers as a result of disease activity and/or treatment [ 101 , 102 ]. The increased intrahepatic frequency of Tregs in AIH patients reported by Peiseler et al [ 82 ] and Taubert et al [ 83 ] is in agreement with our data showing increased numbers of Tregs in the liver of Traf6∆TEC mice.…”
