Future of Digital Assays to Resolve Clinical Heterogeneity of Single Extracellular Vesicles

Abstract: Extracellular vesicles (EVs) are complex lipid membrane vehicles with variable expressions of molecular cargo, composed of diverse subpopulations that participate in the intercellular signaling of biological responses in disease. EV-based liquid biopsies demonstrate invaluable clinical potential for overhauling current practices of disease management. Yet, EV heterogeneity is a major needle-in-a-haystack challenge to translate their use into clinical practice. In this review, existing digital assays will be di… Show more

“…The development of TENPO to isolate EV subpopulations from clinical specimens offers new opportunities in biomarker development and understanding EV biology. As EV-based diagnostics move towards clinical application, the literature has shown how EV heterogeneity drives cancer biology [34], in uences diagnostics [35][36], and modulates cargoes in different EV types [37]. Using immunomagnetic isolation via speci c surface markers, TENPO can take advantage of EV heterogeneity by sorting distinct EV subpopulations from a single patient sample.…”

“…Moreover, the clinical practice of EVs is challenged by their heterogeneity to some extent. Optimization of current digital assays has been proposed to do single-vesicle-resolution studies, especially during the clinical analysis of liquid biopsies ( 215 ). Immune biocompatibility of EVs is the other criterion to be investigated in autoimmune diseases.…”

“…EVs were reported to demonstrate cell and tissue-specific autonomous targeting capabilities ( 139 , 215 ). The interactions between EVs and the neighboring or distant acceptor cells occur through diverse mechanisms.…”

“…Then signals are amplified by chemical or enzymatic reactions and reported via fluorescence within each complex. [237,238] Digital assays characterize single EVs with higher accuracy than analog measurements. Two DMF for single EV characterization will be discussed in this article, including dELISA and dPCR.…”
