Future Directions of Neural Monitoring in Thyroid Surgery

Abstract: Dissecting and identifying the recurrent laryngeal nerve (RLN) are considered routine procedures now that safe and effective methods have been established. Preventing RLN injury during thyroid surgery requires good visualization and exposure of the RLN, adequate knowledge of RLN anatomy, adequate surgical experience and training, and pre-and postoperative laryngoscopy. Whereas these requirements are widely accepted for routine thyroid surgery, new technical developments have emerged in the past 15 years. Liter… Show more

“…However, several studies found the overall incidence of RLN injury of 3.1–5.9% for TOETVA to be comparable to the incidence of 2.1–11.8% for open thyroidectomy [ 16 , 21 , 22 , 23 ]. The low incidence of RLN injury in TOETVA can be explained by the reduced incidence of traction injury and the use of IONM [ 23 , 24 ]. During TOETVA, the RLN is usually identified at its insertion site and released first after division of the Berry’s ligament.…”

“…cIONM has been shown to be feasible, reliable, and safe for monitoring RLN status during thyroid surgery (7). Kim et al have discussed in depth the two different techniques, along with their advantages and limitations, and recommended future directions (8).…”
