Purpose: Gut microbiota have been implicated in the development of colorectal cancer. We evaluated the utility of fecal bacterial marker candidates identified by our metagenome sequencing analysis for colorectal cancer diagnosis.Experimental Design: Subjects (total 439; 203 colorectal cancer and 236 healthy subjects) from two independent Asian cohorts were included. Probe-based duplex quantitative PCR (qPCR) assays were established for the quantification of bacterial marker candidates.Results: Candidates identified by metagenome sequencing, including Fusobacterium nucleatum (Fn), Bacteroides clarus (Bc), Roseburia intestinalis (Ri), Clostridium hathewayi (Ch), and one undefined species (labeled as m7), were examined in fecal samples of 203 colorectal cancer patients and 236 healthy controls by duplex-qPCR. Strong positive correlations were demonstrated between the quantification of each candidate by our qPCR assays and metagenomics approach (r ¼ 0.801-0.934, all P < 0.0001). Fn was significantly more abundant in colorectal cancer than controls (P < 0.0001), with AUROC of 0.868 (P < 0.0001). At the best cut-off value maximizing sum of sensitivity and specificity, Fn discriminated colorectal cancer from controls with a sensitivity of 77.7%, and specificity of 79.5% in cohort I. A simple linear combination of four bacteria (Fn þ Ch þ m7-Bc) showed an improved diagnostic ability compared with Fn alone (AUROC ¼ 0.886, P < 0.0001) in cohort I. These findings were further confirmed in an independent cohort II. In particular, improved diagnostic performances of Fn alone (sensitivity 92.8%, specificity 79.8%) and four bacteria (sensitivity 92.8%, specificity 81.5%) were achieved in combination with fecal immunochemical testing for the detection of colorectal cancer.Conclusions: Stool-based colorectal cancer-associated bacteria can serve as novel noninvasive diagnostic biomarkers for colorectal cancer.

Efecto antimicrobiano aceite esencial de Citrus reticulata 7 Efecto antimicrobiano del aceite esencial de Citrus reticulata sobre Fusobacterium nucleatum asociada a enfermedad periodontal Antimicrobial effect of essential oil of Citrus reticulata on Fusobacterium nucleatum associated with periodontal disease RESUMEN La clorhexidina como tratamiento de la enfermedad periodontal ha logrado efectos bactericidas sobre periodontopatógenos y biopelícula oral. Su uso genera efectos adversos, por lo tanto se presentan alternativas naturales con efecto antimicrobiano similar. Los aceites esenciales han demostrado efectividad en el control de la placa dental, sin los efectos adversos de la clorhexidina. El objetivo de este estudio fue determinar el efecto bacteriostático y bactericida del aceite esencial de mandarina contra Fusobacterium nucleatum. Se realizó extracción por expresión del aceite esencial de cáscaras de mandarina (variedades Arrayana y Oneco). Se evaluaron concentraciones al 20%, 40%, 60%, 80% y 100% del aceite esencial diluido en Tween al 0,02%. El efecto bacteriostático y bactericida se determinó por pruebas de sensibilidad antimicrobiana por difusión en disco. Como control positivo se utilizó Clorhexidina 0,2% y agua como control negativo. Se midió halo de inhibición (mm) y se determinó ausencia o presencia de crecimiento bacteriano a partir de unidades formadoras de colonias. Para comparación de proporciones de la actividad bacteriostática y bactericida, se realizó prueba de Fisher y T student (IC 95% p = 0,05). El halo de inhibición a una concentración del 100% mostró comportamiento similar a clorhexidina (p<0,05). Concentraciones al 100% y 80% fueron bactericidas, al 60%, 40% y 20% presentaron comportamiento bacteriostático. No se encontraron diferencias significativas en las proporciones de inhibición entre las dos variedades de mandarina (p>0,05). El uso de aceites esenciales de mandarina podría ser una alternativa complementaria al tratamiento de la enfermedad periodontal.Palabras clave: aceite esencial, agente antimicrobiano, cítrico, cavidad oral, odontopatógenos ARTÍCULO DE INVESTIGACIÓNRev.

Colorectal cancer may develop slowly over years from precursor lesions, and thus screening combined with early diagnosis is the key to disease prevention. Recent studies have elucidated specific traits in the gut microbiome associated with colorectal cancer and suggested that the microbiome may be useful in screening for colorectal cancer purposes but failed to provide protocols that can be applied in a practical situation. A recent study by Zackular and colleagues, presented on page 1112, provides an important way forward here in showing that specific analysis of multiple aspects of the microbiome composition in toto provides reliable detection of both precancerous and cancerous lesions. This important achievement when combined with other noninvasive techniques promises to provide highly effective tools for early colorectal cancer diagnosis and its prevention.
