Total anomalous pulmonary venous drainage is a rare congenital anomaly. It usually involves a pulmonary to systemic venous shunt and most cases have a septal defect in order to survive. Anomalous pulmonary venous drainage with pulmonary venous shunting is an extremely rare and entirely benign entity. We present two such cases, in which there was atresia of the left superior pulmonary vein and drainage via a tortuous collateral vein to the left inferior pulmonary vein. This collateral was mistaken on plain film and CT for a pulmonary arteriovenous malformation. Awareness of this anomalous unilateral single pulmonary vein and its radiological appearances may help in avoiding unnecessary pulmonary angiography.

“…Der Zusammenhang zwischen der Entstehung einer varikösen Erweiterung der Pulmonalvenen und einer Mitralinsuffizienz wird durch die Beobachtung unterstrichen, dass sich die pulmonalvenöse Erweiterung nach Mitralklappenersatz zurückbildete [31,32]. In einigen Fällen wird eine paravalvuläre Leckage nach Mitralklappenersatz mit entsprechender Rückwirkung auf den Vorhof-und den Pulmonalvenendruck und Ausbildung einer lokalisierten Venenerweiterung beschrieben [33 ± 35], wobei die Erweiterung der Vene als ein erstes Hinweiszeichen auf eine postoperative Komplikation bewertet wird [33,34]. Auch eine relative Mitralinsuffizienz bei Aortenklappeninsuffizienz kann zur Ausbildung und zu einer Gröûenzunahme eines Pulmonalvenenvarix führen, wie sie bei einem Zustand nach Aortenisthmusstenose zehn Jahre nach operativer Sanierung beobachtet wurde [36].…”

ZusammenfassungVarizen der Pulmonalvenen sind eine sehr selten beobachtete Entität. Neben kongenitalen Varizen, die meist kombiniert mit anderen Missbildungen im Bereich der Pulmonalgefäûe auftreten, sind Varizenbildungen vor allem im Zusammenhang mit einer Mitralinsuffizienz beschrieben worden. Die Diagnose von Varizen der Pulmonalvenen erfolgt meist zufällig, da sie symptomlos sind bzw. nur gelegentlich zu einer hinweisenden Symptomatik führen. Aufgrund des radiologischen Befundes müssen alle Differenzialdiagnosen der pulmonalen Rundherde diskutiert werden. Wir stellen unsere eigene Beobachtung vor, um auf die diagnostischen Erfordernisse (CT, Multi-Slice-CT) und auf differenzialdiagnostische Aspekte hinzuweisen. AbstractPulmonary varicosity is a very rare entity. Beside congenital varices, most combined with other malformations of the pulmonary vessels, varicosities are described above all in connection with mitral insufficiency. Most cases with pulmonary varicosity are diagnosed by chance, because of absence of any symptoms or only slight symptoms respectively. We demonstrate our own case with respect to the diagnostic requirements (computerized tomography, multi-slice CT) and the differential diagnostic aspects.
