Introduction: a,-antitrypsin and the serpins The maintenance of health requires a balance between the defensive and offensive systems ofthe body. One such balance that is now just becoming understood at the molecular level is the one between the enzymes that break down protein and the antiproteases that act as their inhibitors. Of particular importance is the group of closely related enzymes, the serine proteases, which are responsible for triggering the inflammatory cascades of the plasma such as coagulation, kinin release, fibrinolysis, and complement activation. The triggering by these enzymes of the plasma cascades is controlled by an appropriately specialized group of plasma protease inhibitors, e.g., antithrombin, antiplasmin, and the first component ofcomplement (C1)' inhibitor.

The immune system is central in the body's defense against non-self. Immunoglobulins and acute phase proteins have been reported to play active roles in carcinogenesis. This prospective longitudinal study was carried out to determine the state of humoral immunity in Nigerian oral cancer patients relative to controls. Twenty newly diagnosed untreated cases of oral squamous cell carcinoma recruited from our centre were included in the study. The controls included 20 apparently healthy and HIV negative volunteers. Serum immunoglobulin classes and acute phase proteins were measured using immunoplates. IgA and ceruloplasmin showed statistically significantly elevated levels in the patients compared with controls, while increases in IgM and IgG were insignificant. The raised levels suggest a role for immunoglobulin A and ceruloplasmin in the mechanisms involved in oral cancers. Findings from this study are similar to that reported elsewhere in the literature. Further work is needed to ascertain the role and usefulness of immunoglobulins and acute phase proteins in staging, disease monitoring, therapy and prognostication.

The proliferation of normal cells, and of early-passage cells from tumours, is inhibited as a consequence of inhibition of a cell-surface proteinase activity. In contrast, established tumour cell lines are usually resistant to these effects. It has previously been suggested that the change from sensitivity to resistance may be a consequence of tumour progression, but experiments with an additional three human tumour cell cultures show that this change is not inevitable.
