Fungal liver abscess in an immunocompetent patient who underwent repeated ERCPs and subtotal cholecystectomy

Abstract: We report a case of a previously healthy female patient who initially presented with fever, jaundice and right upper quadrant pain three days after dilatation and stenting of a stricture of the common bile duct (CBD). During an earlier admission, the patient had undergone endoscopic retrograde cholangiopancreatography (ERCP) having presented with fevers and biliary dilatation on ultrasound. The ERCP features were more consistent with Mirizzi's Syndrome. The patient subsequently underwent subtotal cholecystecto… Show more

“…Pancreatic fungal infections occur in patients with severe acute pancreatitis, 7 a history of surgical manipulation of the abdomen after an episode of acute pancreatitis, ERCP, 8 total parenteral nutrition, and mechanical ventilation. 9 An altered host immune system due to chemotherapy, steroid therapy, comorbid diabetes mellitus, and advanced age is also a predisposing risk factor for pancreatic fungal infections.…”

“…However, the causative species was Candida dubliniensis in this case [ 9 ]. Menachery et al and Hasan et al both reported a solitary C. albicans liver abscess in their patients [ 10 , 11 ]. However, the patient reported by Hasan et al developed the liver abscess primarily as a complication of repeated endoscopic retrograde cholangiopancreatography (ERCP) and subtotal cholecystectomy [ 11 ].…”

However, the patient reported by Hasan et al developed the liver abscess primarily as a complication of repeated endoscopic retrograde cholangiopancreatography (ERCP) and subtotal cholecystectomy [ 11 ]. Furthermore, as in our patient, respiratory symptoms as the primary presenting complaint have not been documented before, and thus physicians should remain cognizant of this unique presentation.…”

“…Vancomycin is typically reserved for the patient in septic shock or cases where staphylococcus aureus may be a concern [30,31]. Fluconazole is commonly added in the rare event that the abscess is of fungal etiology [32].…”
