Aim-To develop and assess a protocol for the treatment of intraocular lymphoma by intravitreal injection of methotrexate and thiotepa. Methods-A patient with intraocular nonHodgkin's lymphoma which recurred after radiotherapy and repeated systemic chemotherapeutic regimens underwent repeated intravitreal injections of methotrexate and thiotepa. The patient was closely monitored by cytology, anterior chamber flare measurements, IL-10 and IL-6 levels. Methotrexate drug clearance studies were performed on vitreous samples taken before each injection. Results-Complete tumour clearance was achieved by the third week of therapy. IL-10 and IL-6 levels quickly dropped to barely detectable levels as the tumour was cleared from the eye. Flare measurements decreased from 500 to 15 photons/s over the same time. A plot of the methotrexate levels over time revealed a first order kinetic rate of elimination with an eVective tumoricidal intravitreal dose persisting for 5 days after injection. Conclusion-Intravitreal chemotherapy for the treatment of recurrent intraocular lymphoma appears eVective in prolonging local remission of ocular disease even in the presence of an aggressively growing tumour. A single intravitreal injection of methotrexate can lead to a prolonged tumoricidal concentration lasting for a longer period than that achieved by systemic administration. (Br J Ophthalmol 1999;83:448-451) Primary central nervous system (CNS) and intraocular non-Hodgkin's lymphoma (NHL) is initially treated with systemic chemotherapy administered alone or in combination with radiotherapy.

Zusammenfassung.Hintergrund: Das gleichzeitige Auftreten zweier unterschiedlicher Tumore mesenchymaler Genese, stellt den Verdacht, dass die Ursache eine direkte Storung der Mesenchymentwicklung sein konnte. Der haufigste pathogenetische Faktor eines malignen Histiozytoms ist, neben chemischen Stoffen and Viren, radioaktive Strahlung.Material and Methoden: Ein 48 jahriger aus Ost-Kasachstan stammender Patient stellte sich wegen Diplopie mit anhaltenden Kopfschmerzen vor. Bei einem 40 jahrigen Patienten zeigte sich rechtsseitig im temporalen Orbitarandbereich subkutan ein derber Tumor. Es erfolgte eine Resektion sowie eine histologische and immunhistochemische Untersuchung.Ergebnisse: Die ophthalmologische Untersuchung des zuerst genannten Patienten zeigte einen Exophthalmus mit Hypo-and Exotropie. Das CT zeigte eine Raumforderung im Bereich der Orbita links sowie eine zweite okzipital. Therapeutisch erfolgten Tumorresektionen. MRT-Untersuchungen zeigten uber einen Zeitraum von bislang 1,5 Jahren postoperativ keine Tumorreste bzw. Rezidive. Die histologische Untersuchung des orbitalen Tumors in Kombination mit einer Immunhistochemie zeigte ein benignes Histiozytom, die Histologie des okzipitalen Tumors ein fibromatoses Meningeom.Die Sehscharfe and der Augenbefund des zweiten Patienten waren beidseits unauffallig. Die Histologic des resezierten Tumors zeigte ein fibroses Histiozytom. Nach 4 Monaten wurde ein Rezidiv festgestellt. Eine breitere Resektion fuhrte in einem Zeitraum von bislang 10 Jahren zur Rezidivfreiheit.Schlussfolgerung: Die Entstehung eines fibrosen Histiozytoms komrte mit einer erhohten Strahlenbelastung in Verbindung gebracht werden. Die Lokalisation benigner Tumore in der Orbita entscheidet uber die Aggressivitat ihres Wachstums. Die nasale Lokalisation scheint dabei besonders gefahrlich. Schlusselworter: Intraorbitales fibroses Histiozytom, mesenchymale Tumore, Radiatio. Teile dieser Arbeit wurden als Vortrag im Rahmen der 53. The most frequent pathogenetic factor is beside viruses and chemical materials radiation.Material and methods: A 48-year old patient coming from Kasachstan introduced himself with diplopia and continuous headache. In a 40-year old patient a crude tumor showed up in the right temporal orbita boundary region. A resection as well as a histological and immunohistochemical investigation took place.Results: The ophthalmological investigation of the patient specified first showed an exophthalmus with hypo and exotropia.
