Functional vision with cataracts of different morphologies: Comparative study

Abstract: The VF-14 questionnaire reliably evaluated functional differences caused by different cataract morphologies; these differences were underestimated when only visual acuity was measured. Patients with PSC had increased functional impairment, indicating that cataract surgical intervention is indicated at an earlier stage in these patients. The significant differences between the morphological types of cataract should be taken into consideration when the benefit of cataract surgery is to be measured on the basis o… Show more

“…12 It has been proposed that age-related lens sclerosis may play a role in this decrease. 13,14,15 One of the most comprehensive of these studies is that of Owsley et al, 16 which included 91 subjects. They observed decreased contrast sensitivity at high spatial frequencies but found no effect at lower frequencies in subjects over 40 years old; they also noted that small children had high contrast sensitivity at low frequencies, but low sensitivity at intermediate and high frequencies.…”

“…The mechanism of cataract induction involves the blockage of swelling-activated chloride channels in the lens,201, 202 and it is at least partly independent of the actions of tamoxifen on ERs 203,204. The increased risk of posterior subcapsular cataracts may be as high a factor of 4,169 which is important because posterior subcapsular cataracts tend to impair function more than other types of cataracts 205. This visual impairment may adversely affect cognitive test performance, even at a time when visual acuity is affected minimally 206…”

“…17 Consequently, it has been suggested that VA is not the most appropriate tool to investigate the impact of symptomatic non-advanced cataract on visual function and experience, 11 and complementary and/or alternative techniques that have been investigated include CS, 18 glare disability (GD), 19 reading performance, 20 and functional visual difficulty assessed by various questionnaires. [21][22][23] Morphological and optical characteristics of lens opacification are also important, and can be assessed by various grading systems 24 and Scheimpflug photography, 25,26 respectively, and have the advantage of being independent of patient cooperation and ocular co-morbidity.…”
