Functional Significance of KCNH2 (HERG) K897T Polymorphism for Cardiac Repolarization Assessed by Analysis of T‐Wave Morphology

Abstract: The common K897T polymorphism of the cardiac potassium channel KCNH2 has functional significance for cardiac electrical properties. Subjects with a less common genotype, KT or TT, have smaller TCRT, which reflects dyssynchrony between depolarization and repolarization and is associated with an increased risk of cardiac mortality.

“…The two SNPs that appear to have the strongest connection to (patho)physiological phenotypes are rs1805123 in exon 11 and rs1036145 in intron 2. The minor allele in rs1805123 results in residue 897 changing from lysine to threonine (K897T) and has been identified in genome-wide association studies as a significant determinant of QT interval duration (126,374,397,444,496). The minor allele in SNP rs1036145 is significantly correlated with expression of the KCNH2-1c isoform, which is correlated with development of schizophrenia (279).…”

Subjects with a less common genotype had smaller spQRSTa values which reflects desynchronization between depolarization and repolarization and is associated with an increased risk of cardiac mortality [63]. …”

“…For each ECG, the digital signal of one beat of each lead was used. Several descriptors of T-wave and QRS complex morphology were automatically calculated from the 12-lead ECGs, using a custom-made software application [15,25]. The maximum amplitude of the T-wave is defined as the difference between the peak of the T-wave and the baseline.…”

“…The QT interval was also measured in every lead of the surface ECG and QT dispersion was calculated using the difference between the maximum and minimum of the QT interval in any of the leads. The ECGs were scanned and digitised using UN-SCAN-IT Graph Digitising System Version 6.0 (Silk Scientific, Orem, UT, USA) [15,25]. For each ECG, the digital signal of one beat of each lead was used.…”
