Functional Recovery in Critically Ill Children, the “WeeCover” Multicenter Study

Abstract: This study provides new information regarding functional outcomes and the factors that influence meaningful aspects of functioning in critically ill children. Identifying patients at greatest risk and modifiable targets for improvement in PICU care guides us in developing strategies to improve functional outcomes and tailor to the rehabilitation needs of these patients and their families.

“…Caregiver stress was associated with higher levels of dissatisfaction with their child's home participation. This is consistent with prior literature that found caregiver stress is associated with other poor outcomes for children, such as hospital length of stay for the PICU population and for poor recovery in other populations [4,22,39]. It is also possible the stress itself makes it difficult for caregivers to be satisfied with their child's current participation levels, or that their stress is a result of their child's participation not meeting their expectations.…”

“…15 However, in recent years, both the number of critically ill patients and that of patients requiring readmission to intensive care units (ICUs) has increased. [16][17][18][19] These additional demands on limited intensive care resources have encouraged the application of noninvasive respiratory support methods in non-ICU settings including paediatric wards, emergency rooms (ERs) and during transport. Moreover, the implementation of high-dependency or step-down units with the necessary resources to safely deliver and monitor respiratory support has also assisted in reducing the need for ICU admission.…”

“…Both short and long hospitalizations can lead to a dramatic change in the functioning of the child and his or her parents. 7,8 Often, new diagnoses emerge and the extent of recovery may be unknown.…”
