Functional Recovery after Operative Lower Limb Lengthening at High-Division Regime in Children

Abstract

Цель. Изучить особенности функциональной реабилитации детей с приобретенными и врожденны-ми укорочениями нижних конечностей после оперативного удлинения с применением различных режимов дистракции и условий остеосинтеза.Материал и методы. Проведен анализ результатов лечения 95 детей с врожденными и приобретенны-ми укорочениями нижних конечностей методом дистракционного остеосинтеза по Илизарову. Режим дис-тракции: группа 1 (n=27, возраст 9,9±4,1 года) -1 мм в сутки за 4 приема; группа 2 (n=30, возраст 10,1±1,9 года) -1 мм в сутки за 60 приемов в автоматическом режиме; группа 3 (n=38, возраст 11,5±2,3 года) -1 мм в сутки за 60 приемов в автоматическом режиме в сочетании с интрамедуллярным армированием эла-стичными стержнями с гидроксиапатитным покрытием. После окончания аппаратного лечения оценивали восстановление движений в коленном и голеностопном суставах, динамику силы мышц антагонистов и сроки начала ходьбы без дополнительной опоры.Результаты. Показано, что при удлинении костей конечностей в высокодробном автоматическом режиме с использованием роботизированного дистрактора и дополнительным интрамедуллярным арми-рованием стержнями с биоактивным покрытием происходит более раннее восстановление активной и пассивной амплитуды движений в смежных суставах, силы мышц сегмента и полноценной осевой нагрузки конечности. Этот эффект может быть связан как с благоприятными условиями для регенерации костной и мышечной ткани в процессе высокодробной дистракции, так и с возможностью более ранней полной функциональной нагрузки после демонтажа аппарата Илизарова, когда остаются интрамедуллярные им-планты.Заключение. Таким образом, высокодробное круглосуточное удлинение в сочетании с интрамедул-лярным биоактивным армированием создает условия для благоприятного течения реабилитационного пе-риода и для развития мышечного аппарата.
