BackgroundOsteoarthritis (OA) is a common joint disease that causes disabilities in elderly adults. However, few long-lasting pharmacotherapeutic agents with low side effects have been developed to treat OA. We evaluated the therapeutic effects of intra-articular injections of hydrogels containing hyaluronic acid (HA) and doxycycline (DOX) in a rabbit OA model.ResultsThirteen week old New Zealand White rabbits undergone a partial meniscectomy and unilateral fibular ligament transection were administered with either normal saline (NT), HA, DOX or HA-DOX hydrogels on day 0, 3, 6, 9 and 12; animals were also examined the pain assessment in every three days. The joint samples were taken at day 14 post-surgery for further histopathological evaluation. The degree of pain was significantly attenuated after day 7 post-treatment with both HA and HA-DOX hydrogels. In macroscopic appearance, HA-DOX hydrogel group showed a smoother cartilage surface, no or minimal signs of ulceration, smaller osteophytes, and less fissure formation in compare to HA or DOX treatment alone. In the areas with slight OA changes, HA-DOX hydrogel group exhibited normal distribution of chondrocytes, indicating the existence of cartilage regeneration. In addition, HA-DOX hydrogels also ameliorated the progression of OA by protecting the injury of articular cartilage layer and restoring the elastoviscosity.ConclusionOverall, from both macroscopic and microscopic data of this study indicate the injectable HA-DOX hydrogels presented as a long-lasting pharmacotherapeutic agent to apply for OA therapy.

Objective: The aim of this consensus statement on viscosupplementation is to serve as a reference document based on relevant literature and clinical experience in the treatment of knee osteoarthritis using an intra-articular injection of hyaluronic acid, covering key aspects such as clinical indications, effectiveness, and tolerability. Methods: A multidisciplinary panel including two sports medicine physicians, six orthopedists, four physiatrists, and two rheumatologists were selected based on their clinical and academic experience of viscosupplementation. Sixteen statements were prepared and discussed, after which a vote was held. Each member of the panel gave a score between 0 and 10 on a Likert scale, specifying their level of agreement with the statement. Results: The panel reached a consensus on several issues. Specifically, the panel agreed that the best indication is for mild to moderate knee arthrosis; prior or concomitant use of intraarticular triamcinolone hexacetonide may optimize the effect of hyaluronic acid; viscosupplementation should not be performed as an isolated procedure but in conjunction with other rehabilitative and pharmacological measures; viscosupplementation has analgesic, anti-inflammatory, and chondroprotective effects; and viscosupplementation is cost-effective. W S T Ę P:Choroba zwyrodnieniowa stawów kolanowych (gonartroza -GA) należy do najczęstszych schorzeń narządu ruchu, a ból i ograniczenie ruchomości kolana są najdotkliwiej odbieranymi przez pacjentów objawami zmian zwyrodnieniowych. Celem pracy było sprawdzenie, czy podanie dostawowe preparatu kwasu hialuronowego wpływa na funkcje stawu kolanowego, wybrane parametry układu antyoksydacyjnego i natężenie stresu oksydacyjnego we krwi u pacjentów z gonartrozą. M A T E R I A Ł I M E T O D Y:Grupa badana 1K obejmowała 96 pacjentów z chorobą zwyrodnieniową stawu kolanowego, którym podano kwas hialuronowy do jednego stawu kolanowego, natomiast do grupy badanej 2K włączono 33 pacjentów, którym podano kwas hialuronowy do obu stawów kolanowych. Badanie prowadzono przez 40 tygodni według ustalonego protokołu. Oznaczono całkowity status oksydacyjny (TOS) osocza, zawartość grup sulfhydrylowych (SH) w surowicy, aktywność katalazy (CAT) w erytrocytach, aktywność dysmutazy ponadtlenkowej (SOD) w osoczu i erytrocytach, aktywność peroksydazy glutationowej (GPx) w erytrocytach. W Y N I K I: Po leczeniu wiskosuplementacyjnym odnotowano zmniejszenie nasilenia bólu oraz poprawę w badanych skalach VAS i HHS, przy czym nieco większą poprawę stwierdzono w przypadku zajęcia jednego kolana. Po leczeniu dostawowym kwasem hialuronowym stwierdzono znamienny spadek aktywności SOD oraz CAT a wzrost aktywności GPx, wzrost stężenia grup SH w obu grupach oraz spadek stężenia TOS. WNIOSKI: Wiskosuplementacja w chorobie zwyrodnieniowej stawu kolanowego istotnie redukuje ból kolana i poprawia jego funkcje oraz wywołuje korzystne zmiany w układzie antyoksydacyjnym krwi. Efekt leczenia jest porównywalny zarówno w przypadku podawania preparatu kwasu hialuronowego do jednego, jaki i do obu stawów kolanowych.
