Objective To determine pregnancy outcome in women with atrial switch repair for transposition of the great arteries (TGA) and to compare follow up with a control group of childless women with the same repair.Design Retrospective cohort study.Setting Tertiary care medical centre.Population About 21 patients compared with 15 controls.Methods Review of records from joint cardiac-obstetric clinic 1993-2013.Main outcome measures Occurrence of cardiovascular events: maternal death, heart failure, arrhythmia, thromboembolic events, worsening systemic ventricular function, worsening tricuspid valve regurgitation and newly detected baffle problems.Results There were 34 pregnancies in 21 women. Mean follow up was 100 months. No deaths or recurrence occurred. Events (few arrhythmias, thromboembolic events and baffle issues) were common in both groups: 13 (62%) patients and eight (53%) controls (P = 0.736). Worsening of ventricular function was similar in both groups: six (29%) patients and four (27%) controls (P = 0.899). Worsening tricuspid regurgitation was more common in patients [11 (52%)] than controls (0)] (P < 0.001). Labour was induced in 76% cases: 32% for cardiac deterioration, 37% to control timing of delivery, and 26% for intrauterine growth restriction. Delivery was vaginal in 84% cases. Median gestational age was 37 (30-40) weeks, median birthweight 2525 g (1460-3530). In all, 38% babies were premature and 38% were small-for-gestational-age.Conclusions Cardiac events after atrial repair for TGA are equally common in pregnant women and non-pregnant controls, although worsening tricuspid regurgitation occurs more frequently in pregnancy. Induction of labour is to be expected but vaginal delivery is achievable in most cases. As surgical techniques continue to develop, for example as seen with the Norwood procedure for hypoplastic left heart syndrome, the number of patients with the most complex forms of CHD will continue to increase along with their associated long term complications. Our ability to manage these patients appropriately, therefore, must also develop alongside. The purpose of this review is to examine the management of these patients once they have developed advanced disease, including drug therapy, devices, transplant and end of life care. In particular, the unique problems posed by patients with a systemic right ventricle (RV) and univentricular circulations are discussed. DRUG THERAPY IN HEART FAILUREIn patients with complex congenital heart disease, either palliated or unoperated, the development of ventricular dysfunction and clinical heart failure are common and as discussed above, is a common cause of morbidity and mortality in this population.

Bevezetés: A teljes nagyér-transzpozíció az egyik leggyakoribb, cyanosissal járó veleszületett szívfejlődési rendellenesség. Célkitűzés: A szerzők célja a teljes nagyér-transzpozíció miatt Senning-és Mustard-féle műtéten átesett betegeik hosszú távú túlélésének, funkcionális stádiumának, aritmológiai kontrolljának és életminőségének elemzése és össze-hasonlítása volt. Módszer: A vizsgálatban 85, teljes nagyér-transzpozíció miatt operált beteg vett részt, közülük 37 esetben Senning-műtét, míg 48 esetben Mustard-műtét történt. Valamennyi esetben utánkövetéses vizsgálatot vé-geztek. Eredmények: A több évtizedes utánkövetés sikeraránya 74%-nak bizonyult. A 31 utánkövetett Senning-műtött közül 12-en hunytak el a követés során (39%), míg a 32 Mustard-műtött közül 16-an haláloztak el (50%) (p = 0,45). Nem volt különbség a szívelégtelenség és az aritmiák jellemzői között sem a két betegcsoportban. Az életminőséget és a funkcionális kapacitást jellemző paraméterek kedvezőbbnek bizonyultak a Senning-műtött betegekben. Követ-keztetések: A Mustard-és Senning-műtött TGA-betegek mortalitásában és morbiditásában szignifikáns különbség nem volt igazolható. Aim: The present study aimed to examine and compare long-term survival, functional grading, arrhytmologic and quality of life control in patients with transposition of the great arteries following Senning-and Mustard-operations. Methods: The present study comprised 85 patients with transposition of the great arteries, of whom Senning-operation was performed in 37 cases and Mustard-operation in 48 subjects. Follow-up study was performed in all cases. Results: The success rate of long-term follow-up proved to be 74%. Twelve out of the 31 Senning-operated and 16 out of 32 Mustard-operated patients died during the follow-up (39% vs. 50%, p = 0.45). Neither features of heart failure, nor those of arrhythmias showed differences between the groups, but parameters of quality of life and functional capacity proved to be favourable in Senning-operated patients. Conclusions: There is no significant difference in mortality and morbidity of patients with transposition of the great arteries following Mustard-and Senning-operations. Regarding to long-term follow-up quality of life and functional capacity of Senning-operated patients were more favourable.
