Fulminant hepatic failure secondary to acetaminophen poisoning: A systematic review and meta-analysis of prognostic criteria determining the need for liver transplantation

Abstract: Presently, available criteria are not very sensitive and may miss patients requiring transplantation. Future studies should further evaluate the efficacy of the APACHE II criteria.

“…The KCC demonstrate high specificity, but have been criticised for their low sensitivity and negative predictive value for a poor outcome 12,13 . Nevertheless these easy-to-use criteria have consistently outperformed novel scoring systems and remain the benchmark for prognostication in ALF.…”

“…Another meta-analysis performed to evaluate King's College criterion among patients with ALF, secondary to paracetamol, revealed sensitivity of 69% and specificity of 92%, but Q was 0.61 18 . To overcome these limitations, other alternative prognostic systems might be used 6 .…”

“…Recently, the Acute Physiology and Chronic Health Evaluation (APACHE Ⅱ) score was described for determining prognosis in the setting of FHF. The APACHE Ⅱ used a combination of both clinical and laboratory parameters and its sensitivity was comparable to that of the King's College Criteria [39] . A recent study in 120 consecutive patients with FHF investigated the prognostic efficacies of King's College criteria, Clichy's criteria, the Model for End-Stage Liver Disease (MELD), and the Pediatric End-Stage Liver Disease (PELD).…”
