Frontline Management of Epithelial Ovarian Cancer—Combining Clinical Expertise with Community Practice Collaboration and Cutting-Edge Research

Abstract: Epithelial ovarian cancer (EOC) is the most common histology of ovarian cancer defined as epithelial cancer derived from the ovaries, fallopian tubes, or primary peritoneum. It is the fifth most common cause of cancer-related death in women in the United States. Because of a lack of effective screening and non-specific symptoms, EOC is typically diagnosed at an advanced stage (FIGO stage III or IV) and approximately one third of patients have malignant ascites at initial presentation. The treatment of ovarian … Show more

“…Ovarian cancer (OC) is the fifth leading cause of cancer-related mortality globally, among which epithelial OC is the most common type with a high mortality [ 1 ]. Although OC is relative rare among gynecological malignancies, women with OC are usually diagnosed late due to the nonspecific symptoms and lack of the screening test for the disease [ 2 4 ]. Surgical resection is preferable for women with early stage OC; while for most cases of advanced cancer, tumor debulking followed by adjunctive therapy could be performed.…”

“…The standard treatment consists of optimal cytoreductive surgery with subsequent treatment with a combination of platinum and taxane-based chemotherapy [10,11]. In general, patients who positively respond to initial chemotherapy, classified as responders, show a favorable prognosis with a median survival rate exceeding four years.…”

“…CRS is an aggressive locoregional treatment involving the resection of the disseminated intra-abdominal disease ( 7 , 8 ). This includes, hysterectomy, bilateral salpingo-oophorectomy, omentectomy as well as additional procedures such as peritonectomies ( 9 ), bowel resections, diaphragm peritonectomy with or without segmental full-thickness diaphragm resection, splenectomy with or without distal pancreatectomy, segmental liver resection, cholecystectomy, partial stomach resection, and partial bladder/ureteral resection ( 10 ). Postoperative residual disease was shown to be a major prognostic factor of overall survival ( 11 13 ), hence the necessity of surgery to be complete.…”
