Hypertension is a major public-health issue. Much consensus has been reached in the treatment, and considerable progress has been made in the field of antihypertensive drugs. However, the standard-reaching rate of blood pressure is far from satisfaction. Considering these data and the seriousness of the effects of hypertension on the individual and society as a whole, both economically and socially, physicians must look for more effective and alternative ways to achieve the target blood pressure. Could treatment of hypertension be improved by insights from traditional Chinese medicine? As one of the most important parts in complementary and alternative therapies, TCM is regularly advocated for lowering elevated blood pressure. Due to the different understanding of the pathogenesis of hypertension between ancient and modern times, new understanding and treatment of hypertension need to be reexplored. Aiming to improve the efficacy of Chinese herbal medicine in treating hypertension, the basis of treatment is explored through systematically analyzing the literature available in both English and Chinese search engines. This paper systematically reviews the trends in emerging therapeutic strategies for hypertension from the perspective of traditional Chinese medicine.

Aqueous-ethanolic extract of Cassia alata (AECal) and its derived fractions obtained through liquid-liquid fractionation were evaluated for their bronchorelaxant, genotoxic, and antigenotoxic effects. Contractile activity of rats' tracheas in the presence of tested materials, as well as its modifications with different inhibitors and blockers, was isometrically recorded. The antigenotoxic potential of AECal was evaluated on cyclophosphamide- (CP-) induced genotoxicity in the rat. Animals were pretreated with the extract, then liver comet assay was performed. AECal and its chloroformic fractions (CF-AECal) relaxed the contraction induced by Ach, but both were significantly less potent in inhibiting contraction induced by KCl (30 mM; 80 mM). Propranolol, indomethacin, L-NAME, methylene blue, and glibenclamide did not modify the relaxant effect of CF-AECal. TEA altered the response of trachea to CF-AECal. CF-AECal caused a rightward shift without affecting the E max in cumulative concentration-response curves of Ach only at low concentrations. In animals pretreated with the extract, the percentage of CP-induced DNA damage decreased. Our results suggest that (1) muscarinic receptors contribute at least in part to the relaxant effects of CF-AECal; (2) CF-AECal interferes with membrane polarization; and (3) AECal is not genotoxic in vivo and contains chemopreventive phytoconstituents offering protection against CP-induced genotoxicity.

Hypertension is a risk factor for cardiovascular disease, which is currently a real public health problem. This disease affects about one billion people worldwide and is responsible for more than 70% of cardiovascular related deaths. Recently, the World Health Organization reported that of the hypertensive cases detected in Congo, only 7% were controlled. Today, there is no lifetime treatment and existing drugs are less accessible by the African population. To treat the disease, the Congolese population uses more the medicinal plants. However, the majority of compounds responsible for the biological activity of these plants are not known. In order to bring out Congolese plants with antihypertensive properties, we focus our interest on Trema orientalis Blume (Canabaceae). An ethanolic extract of the leaves of Trema orientalis was prepared after successive depletion of the organic solvents. Thereafter, a bio-guided fractionation on silica gel of the ethanol extract was carried out. Fractionation monitoring was done by TLC and the results of vasodilating activity measured. The fractions exhibiting the best biological activity allowed a second fractionation process to obtain five fractions which are characteristic of polyphenols, in particular flavonoids, and which exhibited good vasodilating activity on the isolated aorta of rats. Our future work will focus on the identification of these biologically active compounds.
