From the battlefield to main street: Tourniquet acceptance, use, and translation from the military to civilian settings

Abstract: Throughout history, battlefield medicine has led to advancements in civilian trauma care. In the most recent conflicts of Operation Enduring Freedom in Afghanistan/Operation Iraqi Freedom, one of the most important advances is increasing use of point-of-injury hemorrhage control with tourniquets. Tourniquets are gradually gaining acceptance in the civilian medical world—in both the prehospital setting and trauma centers. An analysis of Emergency Medical Services (EMS) data shows an increase of prehospital tour… Show more

“…Efforts to publicize awareness about tourniquet use between civilians have increased in the last decades. Pre-hospital tourniquet application to people exposed to peripheral exsanguination harm was found to reduce the mortality rate in these patients ( Goodwin et al, 2019 ). However, measures to avoid the possible deleterious effects resulting from I/R upon tourniquet application must be taken into consideration.…”

“…Enhancing educational opportunities offered by an AMC by including comprehensive mental health content, thoughtful discussions about sources of distress, recognizing emerging pathology, and the importance of identifying beneficial strategies and behaviors are all well suited to healthcare curricula and research programs. In addition, offering the option of a medical history curriculum that highlights medical advances inspired by the military (Goodwin et al, 2019) will demonstrate the institution's tangible awareness of contributions by the military to healthcare (Gibbs, Lee, & Ghanbari, 2019).…”

“…Hemorrhage is the leading cause of preventable death in both military and civilian trauma. Early application of extremity tourniquets at the point of injury is currently accepted as the standard treatment for life-threatening extremity hemorrhage [2][3][4]. The classic Pre-Hospital Emergency Care (PHEM) paradigm changed from airway-breathing-circulation (ABC) to Military-PHEM (M-PHEC) that addresses rst catastrophic hemorrhage, before airway, breathing or circulation (<C> ABC) [5].…”
