From Evidence to Clinical Guidelines in Antibiotic Treatment in Acute Otitis Media in Children

Abstract: Acute otitis media (AOM) in children represents a public health concern, being one of the leading causes of health care visits and antibiotic prescriptions worldwide. The overall aim of this paper is to unravel the major current insights into the antibiotic treatment of AOM in children. Our approach is three-fold: 1. a preclinical evaluation of antibiotics in animal models of AOM stressing on the advantages of different species when testing for different schemes of antibiotics; 2. an overview on the new antimi… Show more

“…The watchful-waiting approach is highly encouraged nowadays in uncomplicated cases of AOM [ 47 ], as in healthy children, the innate immunity is able to manage the AOM episodes without any complications or sequelae [ 48 ]. Recent updates to AOM guidelines attempt to reduce antibiotic overuse even more.…”

“…Especially in respiratory and ear infections-two very frequent indications for antibiotic use in children-anthroposophic physicians tend to prescribe fewer antibiotics [26]. In both types of infections, there is a broad discussion on the advantages of watchful waiting (see [27] and [28], respectively). At least in the case of some types of otitis media, a multimodal approach that includes several non-invasive supportive therapies (pneumatization exercises, education, an antiallergenic diet, nasal hygiene, useful constitutional therapy and thermal interventions; e.g., P.E.A.N.U.T.)…”

“…Furthermore, the high cell density in biofilms has been shown to promote horizontal gene transfer of antimicrobial resistance (AMR) 15 . Despite mechanistic understanding of biofilm resistance to antimicrobials 16 , 17 and the inability of antibiotics to reach concentrations capable of eradicating biofilms in the middle ear 18 , broad-spectrum antibiotics remain the standard of care in pediatric OM due to their short-term efficacy 19 . This practice unquestionably contributes to rapid, global emergence of antibiotic-resistant bacteria 20 , as well as microbiome dysbiosis to the potential detriment of immune development and overall health 4 .…”
