Frequency, Risk Factors, and Outcomes of Early Unplanned Readmissions to PICUs*

Abstract: Objectives To determine the rate of unplanned PICU readmissions, examine the characteristics of index admissions associated with readmission, and compare outcomes of readmissions versus index admissions. Design Retrospective cohort analysis. Setting Ninety North American PICUs that participated in the Virtual Pediatric Intensive Care Unit Systems. Patients One hundred five thousand four hundred thirty-seven admissions between July 2009 and March 2011. Interventions None. Measurements and Main Results… Show more

“…A recent survey suggested that pediatric hospitals vary widely in their use of IMCUs, but that having an IMCU provides an alternative location to the PICU to care for patients needing intense monitoring and nursing care[16]. PICU patients discharged to IMCUs have tended to be “long-stay” patients[17] and had greater odds of early unplanned PICU readmission[18]. Adult studies of the impact of IMCUs on ICU metrics have provided mixed results regarding ICU and hospital LOS[19,20], illness severity in the ICU[5,7,8], and non-emergency ICU admissions[20].…”

“…Unplanned readmissions are a focus of healthcare quality measurement, as they may be preventable (3) and are associated with worse outcomes (45) and higher costs (67). Furthermore, repeated critical illness and hospitalization negatively impact patients and families (810).…”

“…Furthermore, repeated critical illness and hospitalization negatively impact patients and families (810). Except for a few studies focused on early readmissions (4) and readmissions during the same hospitalization (1113), it is not known how often unplanned PICU readmissions occur and who are at most risk.…”

“…A patient's readiness for PICU discharge is determined by a number of different factors, most importantly clinical evaluation by experienced PICU physicians and nurses. Despite best efforts, the incidence of code blue events or urgent need for PICU readmission within 48 hours of PICU discharge remains unacceptably high, with a national average of 2-8% [3][4][5]. Since unplanned PICU readmission has been associated with increased length of PICU stay and higher mortality compared to non-readmitted patients (5,6), a tool to identify this at risk population would be of great clinical utility.…”
