Background: Meningiomas are extra-axial primary neoplasms that frequently arise from arachnoid cap cells. Meningiomas account for around 33% of all primary brain tumours. Objective:The aim of the present study was to compare relationship between different grades of meningiomas and clinicopathological character, treatment and its outcome. Patients and methods: This is a retrospective study included 52 adult meningioma patients treated at Clinical Oncology Department of Zagazig University Hospitals. The study included data of clinicopathological characters, grades of meningioma treatment and its outcome. Results: Among the studied patients, grade I meningioma was more common than grade II and III. The most common site of meningioma was convexity and sphenoid ring. Spinal meningioma accounted for 1.9%. The most common pathological type of meningioma was meningothelial meningioma, transitional and anaplastic. Size of tumor at initial diagnosis was less than 5 cm in 59.5%. Surgical resection was done in all grades of meningioma in 47 patients (90.4%). 33 patients received postoperative radiotherapy (63.5%) and 13 patients were on follow up after surgery (25%). Dose of radiotherapy differed according to grade of meningioma whereas in grade I radiotherapy dose used was (54-55Gy) in 40.5% of patients and dose of 60 Gy was used in patients of grade II and III (42.9% and 75% respectively). Conclusion: Tumor grade was the most important prognostic factor in meningioma patients who received radiation therapy. In patients with grade II and III tumors the extent of surgical resection was significantly associated with over all survive.

Objective : Tumours of the brain are a rare occurrence accounting for approximately 2% of all neoplasms in adults. Few studies have been done in Nigeria on the profile of brain tumours. The aim of this study is to determine the profile of brain tumours in general and determine the change in Kanofsky Performance Score (KPS) after treatment.Methods : This is a prospective hospital-based study in Kaduna. All consecutive patients over 18 years of age with diagnosis of brain tumours from January 2016 to December 2019 were included in the study. Demographic and clinical data was collected using a proforma during the study. Patients who received treatment were followed up for 12 months. The primary outcome data was the difference in the quality of life as measured by KPS at the point of first contact and at 1-month after treatment and at 12-month follow up. Data obtained was analysed with SPSS version 25.0 for Windows. Descriptive statistics was done to determine the profile. Paired t-test at 95% confidence interval was done to check for significant correlation between the mean KPS.Results : A total of 39 consecutive patients were included in the study. There was a slight male preponderance with a M : F of 1.17 : 1. Meningioma and metastasis were more common in females while gliomas and pituitary tumours were more common in males. The mean age of patients was 49.8 years and standard deviation of 11.8 years. Pituitary tumours were the most common tumours. The most common location of the tumour was frontal lobe followed by the pituitary gland. The mean duration of symptoms before neurosurgical consultation was 38 weeks. The most common presenting symptoms of patient with brain tumour was headache. The quality of life improve compare to the baseline in 81% of patient at discharge and at 1 year follow up. The overall mortality rate was 25.6%.Conclusion : The most common brain tumour in our study is pituitary tumour. Most patients present late. The most common presenting symptoms is headache. There is significant improvement in the KPS of patients following treatment. The overall mortality rate at 1-year post treatment is 25.6%.

Background: Tumor cell autophagy can influence cellular immunity by participation in the recognition and modification of tumor-related antigens. Objectives: The objective of this study was to evaluate the immunohistochemical expression of the autophagy-related marker; light chain 3B (LC3B) in tumor cells and the assessment of T lymphocytes by a cluster of differentiation 3 (CD3) in gliomas, and to correlate them with the available clinic-pathological variables in glioma patients. Materials and methods: Immunohistochemical staining for LC3B and CD3 was performed on 60 paraffin-embedded glioma tissue. LC3B immunoreactivity score of 0-6 was designated negative, and those scoring 7-12 were considered positive. The median level of CD3 positive T lymphocytes was calculated for both high and low-grade gliomas. In low-grade gliomas, the CD3 expressing T lymphocytes equal to or more than 2.6, were considered positive while in high-grade gliomas, those equal to or more than 16 were considered positive. Results: LC3B expression in tumor cells was detected in 24/60 (40%) of gliomas. Expression of LC3B was significantly more frequent in high-grade gliomas (23/33, 69.7%) compared to low grade ones (1/27, 5%), (p value= 0.000). LC3B expression was correlated with the patients age (P value= 0.047) & histological variants (P value= 0.000). CD3 positive T lymphocytes were significantly more prominent in high-grade gliomas (25/33 , 41.7%) than low-grade ones (2/27, 3.3%), (P value= 0.001). A significant association was noted between CD3 expression and the patients age (P value= 0.003), sex (P value: 0.035) and histological variants (P value= 0.001). LC3B expression in tumor cells was significantly correlated with CD3 positive T lymphocytes (P value: 0.000). Conclusion: Autophagic activity of tumor cells and T lymphocyte infiltrates were reported more in high-grade gliomas compared to low-grade ones, giving high-grade gliomas the chance in autophagy target therapy & immunotherapy.
