Background Acinetobacter baumannii is one of the most important hospital pathogenic bacteria that cause infectious diseases. The present study aimed to determine the frequency of carbapenem resistance genes in association with transposable elements and molecular typing of carbapenem-resistant A. baumannii bacteria collected from patients in Shiraz, Iran. Materials and Methods A total of 170 carbapenem-resistant A. baumannii isolates were obtained from different clinical specimens in two hospitals. The minimum inhibitory concentrations (MIC) of imipenem were determined and the prevalence of OXA Carbapenemases, Metallo-β-lactamases genes, insertion sequences (IS) elements, and transposons were evaluated by the polymerase chain reaction (PCR) method. Finally, molecular typing of the isolates was performed by the Enterobacterial Repetitive Intergenic Consensus-PCR method. Results The MICs ranged from 16 to 1,024 µg/mL for imipenem-resistant A. baumannii isolates. Out of the 170 carbapenem resistant A. baumannii isolates, bla OXA-24-like (94, 55.3%) followed by bla OXA-23-like (71, 41.7%) were predominant. In addition, A. baumannii isolates carried bla VIM (71, 41.7%), bla GES (32, 18.8%), bla SPM (4, 2.3%), and bla KPC (1, 0.6%). Moreover, IS Aba1 (94.2%) and Tn2009 (39.2%) were the most frequent transposable elements. Furthermore, (71, 44.0%) and (161, 94.7%) of the IS Aba1 of the isolates were associated with bla OXA-23 and bla OXA-51 genes, respectively. Besides (3, 1.7%), (1, 0.6%) and (5, 2.9%) of bla OXA-23 were associated with IS 18 , IS Aba4 , and IS Aba2 , respectively. Considering an 80.0% cut off, clusters and four singletons were detected. Conclusion According to the results, transposable elements played an important role in the development of resistance genes and resistance to carbapenems. The results also indicated carbapenem-resistant A. baumannii bacteria as a public health concern.

Carbapenem-resistant Acinetobacter baumannii are the top urgent antibiotic resistance threat in the world. The aims of this study were the determination of carbapenem-resistant genes and virulence genes among isolates from hospital environments. In this study, A. baumannii isolated from hospital environments and evaluated its antibiotic resistance, virulence factors, and resistance genes. Of 258 samples, 58 showed growth of the target organism. Antibiotic susceptibility test results considered all the A. baumannii to be multidrug-resistant isolates with the highest resistance being 36.2% to ciprofloxacin; while the most effective antibiotics with 98.3% susceptibility was piperacillintazobactam. Of these 58 hospital environment isolates, 18 isolates were positive for Metallo betalactamase. Overall, 65% of the isolates from hospital environments had many virulence factors. PCR assays demonstrated the highest and lowest positive results in csgA and cvaC gene among hospital environment isolates. Results indicate that the determination of carbapenem-resistant genes and virulence genes among isolates from hospital environments is very important.

Introduction: After Staphylococcus aureus and Escherichia coli, Pseudomonas aeruginosa is the third cause of hospital-acquired infection (HAI). This bacteria's ability to colonize in different environments, especially in hospitals and biofilm formation, has added to its impact as an HAI. The molecular mechanism of biofilm formation is not well understood, but several genes contribute to this phenomenon. This study investigates the frequency of cbrA, cbrB, phoBR, and ndvB genes in biofilm-forming P. aeruginosa isolates. Methods: Fifty P. aeruginosa clinical isolates were collected from various sources such as urine, ulcer, blood, secretions, and trachea in Milad Hospital, Tehran, from 2017 to 2018. Biofilm formation in the isolates was assessed by the microtiter plate assay, and the frequency of cbrA, cbrB, phoBR, and ndvB genes was investigated by PCR. Results: Among the 50 isolates, 44% were strong biofilm former, 34% moderate biofilm former, 12% weak biofilm former, and 10% did not form biofilms. PCR revealed a frequency of 94% for the cbrA gene, 78% for cbrB, 96% for ndvB, and 48% for phoBR. The coexistence of all four genes was 68% in strong biofilm former isolates, 41% in moderate biofilm former isolates, 37% in weak biofilm former, and zero in the isolates that formed no biofilm. Conclusion: The high frequency of ndvB and cbrA genes and the coexistence of ndvB and cbrB suggest the contribution of these genes in the biofilm formation of P. aeruginosa.
