Purpose This study aimed to explore whether the mean platelet volume/platelet count ratio (MPR) has predictive value for the occurrence and prognosis of acute ischemic stroke (AIS) in hemodialysis patients. Patients and Methods A total of 402 patients undergoing maintenance hemodialysis were screened and 259 were enrolled in this study. The receiver operating characteristic curve and area under the curve (AUC) were used to evaluate the predictive power of the models. The patients enrolled in this study were divided into three groups based on the tertiles of the MPR value (Q1, Q2, and Q3). Kaplan-Meier curves were used to investigate the association between the MPR and AIS-free survival in hemodialysis patients. Chi-square analysis was performed to explore the association between the MPR and AIS outcomes in hemodialysis patients. And the AIS outcome was assessed using the modified Rankin Scale (mRS). Results MPR had a predictive value for the occurrence of AIS (AUC=0.814) in hemodialysis patients with a high sensitivity and specificity. AIS-free survival rates in the MPR Q1, MPR Q2, and MPR Q3 groups were 0.930, 0.701, and 0.360, respectively. The proportion of patients with good outcomes (mRS 0–2) was significantly greater among patients in the MPR Q1-Q2 group than in the MPR Q3 group (0.844 vs 0.745, p <0.001). Conclusion The MPR can be used as a good predictor of AIS in patients undergoing hemodialysis. Patients on hemodialysis with increased MPR levels had a higher incidence of AIS and poorer functional outcomes than those with low MPR levels.

ObjectiveTo examine the association of lactate-to-albumin ratio (LAR) with 30-day and 90-day mortality in patients with cerebral infarction admitted to the intensive care unit (ICU).MethodsIn this retrospective observational study, 1,089 patients with cerebral infarction were recruited. The concentration of blood lactate and serum albumin on the first day of ICU admission were recorded. The relationship between LAR levels and mortality was evaluated through univariate and multivariate Cox regression analyses, four-knot multivariate restricted cubic spline regression, and Kaplan–Meier (KM) curves.ResultsThe overall 30-day and 90-day mortality rates in the entire cohort were 27.3 and 35.8%, respectively. KM analysis revealed a significant relationship between high LAR index and the risk of all-cause mortality (log-rank p < 0.001). Furthermore, multivariate Cox proportional risk analysis showed that the LAR index independently predicted the risk of 30-day mortality (HR: 1.38, 95% CI 1.15–1.64, p = 0.004) and 90-day mortality (HR: 1.53, 95% CI 1.32–1.77, p < 0.001) in the study population. Furthermore, a higher LAR exceeding 0.53 was positively correlated with the risk of 30-day and 90-day mortalities. Subsequent subgroup analyses demonstrated that LAR could predict the primary outcome.ConclusionIn summary, the LAR index is a reliable and independent predictor of increased mortality among critically ill patients suffering from cerebral infarction. Nonetheless, there is a need for additional comprehensive prospective studies to validate these findings.

Objective: To find out the frequency of hypoalbuminemia and mortality rate in cases of acute ischemic stroke. Material and methods: Between April 2020 to October 2020, total 180 patients of acute ischemic stroke, age between 40-60 years of both gender were selected from Department of Medicine, Bahawal Victoria Hospital, Bahawalpur. Type of study was cross sectional. Hypoalbuminemia and mortality rate was assessed. Results: In present study, mean age of the patients was 49.78 ± 4.43 years. Out of 180 patients of acute ischemic stroke, hypoalbuminemia was found in 68 (38%) patients. Mortality rate was 10 (26%) and rest of the patients were alive. Hypoalbuminemia was found in 28 (28.28%) patients and 40 (49.38%) patients respectively in age groups 40-50 years and 51-60 years. Association of hypoalbuminemia with age group was significant (P = 0.005). Conclusion: Results of this study showed a higher rate of hypoalbuminemia and mortality in cases of acute ischemic stroke. Hypoalbuminemia was more common in 5th decade and significant association of hypoalbuminemia with age was noted. Most of the stroke patients were male and hypoalbuminemia was also common in male patients. Significant association of hypoalbuminemia with diabetes mellitus was observed.
