Einleitung Die COVID-19-Pandemie („coronavirus disease 2019“) hat eindrucksvoll gezeigt, dass Infektionskrankheiten enorme Auswirkungen auf das Gesundheitswesen und darüber hinaus haben können. In der Urologie spielen Viren bei spezifischen Entitäten eine Rolle, wo sich der Urologe mit Viruserkrankungen beschäftigen muss. Methodik Diese Übersichtsarbeit hat zum Ziel in der Urologie relevante Virusinfektionen zu beschreiben und insbesondere die Impfprävention hervorzuheben. Es erfolgte eine selektive Literaturrecherche zu den Themen „COVID und Urologie“, „Urogenitale Virusinfektionen“, „Virale urologische Infektionen in der Transplantationsmedizin“ sowie „Impfprävention von Viruserkrankungen“. Ergebnisse Coronaviren sind Viren, die bereits 2‑mal lokale Epidemien verursacht haben (SARS- [„severe acute respiratory syndrome“] und MERS-Epidemie [„middle east respiratory syndrome“]). Die Tatsache, dass die SARS-CoV-2-Erkrankung („severe acute respiratory syndrome coronavirus 2“) auch ohne Symptome ansteckend ist, hat im Wesentlichen zu der raschen Ausbreitung und weltweiten Pandemie geführt. Eine Vielzahl von Viren, die auch eine Virämie induzieren können, wurde im Ejakulat nachgewiesen und wird damit mit einer etwaigen urogenitalen Infektion in Verbindung gebracht. Hierzu zählen u. a. das Mumps‑, Coxsackie-Viren oder Enteroviren. Es wurde auch gezeigt, dass auch eine Zika-Virusinfektion sexuell über die Spermien als Carrier übertragen werden kann. Somit spielen Viren auch eine wichtige Rolle in der Reproduktion. Bei der Nierentransplantation sind Urologen häufig mit viralen Infektionen konfrontiert. Die effektivste Waffe gegenüber Viren stellt die Impfprävention dar. Schlussfolgerung Äthiopathogenetisch ist der Urogenitaltrakt im Rahmen einer Virämie oder über eine Reaktivierung durch eine Immunsuppression am häufigsten mitbetroffen. Therapeutisch kommt der Immunmodulation sowie der Impfprophylaxe eine führende Rolle zu.

Nowadays, the search for etiopathogenetic factors of infertility is an urgent medical and social task. 23 men with chronic abacterial prostatitis in a barren marriage underwent the follow-up. Herpesviruses of types IV–VI were detected in urogenital samples (urethra, ejaculate, prostate secretions) in 8 of 23 men who were treated with valacyclovir (500 mg 2 times a day for 3 months) and interferon α-2b in combination with antioxidants — vitamins E and C (rectal suppositories). Based on the personal experience of the authors, the patients were prescribed the following drug use regimen: 3 million IU twice a day — 10 days, then 3 million IU twice a day — 3 times per week for the next 3 weeks. The remaining 15 patients without any detected viruses were prescribed the following: levofloxacin 500 mg — 30 days, tamsulosin 0.4 mg — 90 days, diclofenac 50 mg — 10 days. By the end of therapy, electron microscopy and virology showed a decrease in the concentration and number of virus-positive urogenital samples, as well as a decrease in the number of spermatozoa with viral capsids. Within 6 months from the therapy initiation, the onset of pregnancy was observed in the married couples of 6 out of 8 men with herpesviruses vs. in 4 out of 11 without viruses (p=0.026). Thus, we obtained the data on the role of herpesviruses of types IV–VI in male infertility. A good reproductive effect was achieved with valacyclovir and interferon α-2b in combination with antioxidants in most patients. Background: Infertility is a global problem that affects more than 15% of couples. The occurrence of this problem is about 50% in males. Viral infections cause infertility in men by different mechanisms. Objectives: This study is designed to evaluate the effects of hepatitis B virus (HBV) and cytomegalovirus (CMV) infection on male fertility and determine the mechanisms involved. Methods: A total of 73 males were included in the present study. They were assigned to the groups healthy fertile (n = 22 volunteers) as the healthy controls, non-infected infertile (n = 27), HBV-infected infertile (n = 14), and CMV-infected infertile (n = 10). The sperm parameters, including count, morphology, progressive motility (PR), and progressive + non-progressive motility (PR + NP), were analyzed in their semen. The semen samples of all studied participants were subjected to polymerase chain reaction to determine the presence of HBV or CMV infection. Mitochondrial membrane potential (MMP) and tumor necrosis factor-alpha (TNF-α) were also assessed. Results: Our findings showed that all sperm parameters, including count, morphology, PR, and PR + NP, decreased in males with HBV and CMV infections compared to the healthy control group (P < 0.01 in all cases). MMP declined in HBV- and CMV-infected males compared to the healthy control group (P < 0.001). Conversely, the TNF-α level of semen in HBV- and CMV-infected men was significantly higher than in the healthy control group (P < 0.001). Conclusions: The results of the present study demonstrated that HBV and CMV infections lead to male infertility through increasing TNF-α and decreasing sperm MMP.
