Frequency and Pathophysiology of Post-Seizure Todd’s Paralysis

Abstract: Todd's paralysis, a neurological abnormality characterized by temporary limb weakness or hemiplegia, typically occurs following a seizure, without enduring consequences. Since limb weakness or hemiplegia can also be a common symptom of an acute ischemic stroke, it is often difficult to diagnose Todd's paralysis in individuals experiencing an acute ischemic stroke if they do not have a pre-existing history of epilepsy. Given that there is a limited understanding of Todd's paralysis, this review discusses the hi… Show more

“… 8 , 16 , 25 Spreading depolarization arising secondary to a seizure may contribute to postictal complications such as migraine and hemiplegia (e.g. Todd’s paralysis 26 ) whereas spreading depolarization preceding a seizure may underlie migraine-related seizures. However, not all seizures are associated with spreading depolarization, and epilepsy patients do not always display peri-ictal spreading depolarization-related deficits.…”

“…In a recent work, NfL detected drug‐induced neurotoxicity and decreased following cessation of treatment 50 . Of note, epileptic seizures can cause a temporary paralysis of one side of the body that mimics the clinical presentation of a stroke, so‐called Todd's paralysis 51 . A Todd's paralysis usually resolves within few days and NfL could help in excluding a stroke as etiology of such a paralysis, that is , low NfL could support a different etiology than stroke.…”
