Intimate partner violence and severe maternal morbidity among pregnant and postpartum women in São Paulo, Brazil J Hum Growth Dev. 2018; 28(2) Open acess AbstractThis article investigates the association between intimate partner violence (IPV) during a current pregnancy and severe maternal morbidity severe maternal morbidity among pregnant and postpartum women cared for in public maternity centers located in São Paulo, Brazil. A total of 109 women who developed Severe maternal morbidity were selected according to criteria adopted by the World Health Organization (WHO). Another 337 women who did not experience any clinical, laboratory or management intercurrences during a current pregnancy and postpartum were selected for the control group. The participants were submitted to a retrospective investigation of IPV using an instrument adapted from the WHO Multi-country Study on Women's Health and Domestic Violence against Women, applied between November 2010 and June 2011. The relationship between the response variable severe maternal morbidity and the exposure variable (IPV) adjusted for the remaining independent variables was assessed through proportions, a chisquare test, a Fisher's exact test, and multiple logistic regression. A prevalence of 12.6% (CI:9.5-15.7) for psychological violence, 7.6% (CI:5.1-10.1) for physical violence and 1.6% (CI:0.4-2.8) for sexual violence were observed during a current pregnancy in both the case and control groups. Although no statistical significance was found between IPV exposure during a current pregnancy and the occurrence of Severe maternal morbidity (p>0.264), we identified factors associated with unfavorable socio-demographic and reproductive conditions in both the women exposed to IPV and those who developed Severe maternal morbidity. Systematic monitoring of Severe maternal morbidity and routine screening of IPV among pregnant women are important measures to reduce maternal morbidity and mortality and to qualify reproductive health care.Keywords: intimate partner violence, battered women, severe acute maternal morbidity, pregnancy complications, maternal health.

ABSTRACT:This study aimed to analyze the situations that precipitate and/or intensify conflicts in the conjugal relationship. Interviews were held with 19 women experiencing conjugal violence. The Discourse of the Collective Subject was used as the method of organization, making it possible to find the central ideas, and gender was used as the analytical framework. Summary: the women report the man's control and domination of the woman, jealousy, the male partner's infidelity, unplanned fatherhood and motherhood, and the use of alcohol and drugs. These situations are related to the construction of the conjugal violence and/or the woman's remaining in the relationship, with preventive actions being essential so as to avoid this phenomenon and its repercussions on the health of the woman and the family. The work of the professional nurse is indispensable in the programs for harm prevention and for the promotion and assistance of the health of women and families, through social and health service means and facilities existing in the community. Tais situações guardam relação com a construção da violência conjugal e/ou permanência da mulher na relação, sendo essenciais as ações de prevenção, a fim de evitar tal fenômeno, bem como suas repercussões para a saúde da mulher e da família. A atuação do profissional enfermeiro é indispensável nos programas de prevenção de agravos, promoção e assistência à saúde das mulheres e famílias, por meio dos equipamentos de saúde e sociais existentes na comunidade. ABSTRACT:Objective: To analyze the association between physical violence by an intimate partner (PVIP) and the inappropriate use of prenatal care services. Methods: A nested cross-sectional study was conducted with 1,026 women, based on data from a prospective cohort study designed to investigate intimate partner violence among pregnant women enrolled in the Family Health Program (PSF) in Recife, Northeastern Brazil. The use of prenatal care services was assessed with basis on the guidelines from the Program for Humanization of Prenatal Care and Childbirth (Brazilian Ministry of Health) and considered the time of the first prenatal care visit and the total number of visits during the pregnancy. Data were collected through two face-to-face interviews (one in the last pregnancy trimester and the other in the postpartum period), using standardized questionnaires and data on Pregnancy Card records. An unconditional logistic regression was performed to estimate the odds ratio (OR) and the 95% confidence intervals to measure the association between an PVIP and the inappropriate use of prenatal care services, using the stepwise method. Results: The prevalence of the inappropriate use of prenatal care services was 44.1% and of an PVIP, 25.6%. In the logistic regression analysis, an intimatePVIP was associated with inappropriate prenatal care (OR = 1.37; 95%CI 1.01 -1.85; p = 0.04) after adjustment by variables confirmed as confounders (parity, alcohol use in pregnancy, and education level). Conclusion: Women who are victims of an PVIP have more chance of receiving inappropriate prenatal care due to late onset of prenatal care, fewer prenatal care visits, or both. Keywords INTRODUCTIONViolence against women during pregnancy is a major public health issue due to the high risk of maternal and neonatal morbidity and mortality 1 . A study by the World Health Organization (WHO) showed that the prevalence of intimate partner violence during pregnancy can vary from 1 to 28% among countries 2 . This study in Brazil indicated that 8% of women in the city of São Paulo and 11% of women in rural Pernambuco reported having experienced some kind of violence during gestation 2 . Prenatal care can prevent maternal mortality as it modifies the course and the prognosis of pregnancy complications 3 and prevents adverse outcomes, such as perinatal and neonatal mortality, congenital syphilis, and low birth weight 4 . In Brazil, the levels of maternal and perinatal morbidity and mortality remain high, which is incompatible with the current level of economic and social growth in the country . Some factors are described as associated with no prenatal care, which include belonging to social segments with lower income, having less formal education, belonging to the black race, being over 35 years old, being multiparous, having depressive symptoms during pregnancy, making abusive use of alcohol and/or illicit drugs, having unstable marital status, unintended pregnancy history, partner's dissatisfaction with pregnancy, and partner's violence [7][...
