La douleur après chirurgie de la cheville ou de l'arrière-pied est majeure. Elle nécessite le recours à une analgésie multimodale associant différentes techniques d'antalgie : analgésie intraveineuse classique et, plus récemment, analgésie locorégionale avec pose de cathéters périnerveux.Pour des raisons médicales (oedème postopératoire, nécessité d'une surveillance cutanée après bloc périnerveux), nous avons été amenés à renoncer à une contention immé-diate par résine pour certains patients. Observant que ces patients se disaient moins douloureux en postopératoire immédiat, nous avons fait cette étude rétrospective.Cette étude a pour objectif de comparer deux séries de patients opérés de la cheville ou de l'arrière-pied : un groupe des patients qui étaient immobilisés immédiate-ment par une botte en résine prenant la cheville à angle droit, et un groupe de patients sans immobilisation immé-diate, avec résine posée après le 3 e jour postopératoire.L'hypothèse était qu'une contention différée de quelques jours par rapport au geste chirurgical permet de réduire les douleurs postopératoires.Les données de la littérature sur le sujet sont inexistantes.Les patients concernés sont exclusivement des patients bénéficiant d'une intervention programmée (arthrodèse tibio-talienne, sous-talienne, ligamentoplastie, prothèse). 42 patients ont étés inclus : 22 dans le premier groupe (avec résine immédiate) et 20 dans le deuxième groupe (résine différée).Nous avons montré que la mise en place différée d'une résine (au troisième jour), par comparaison avec une résine postopératoire immédiate, permet de diminuer les consommations morphiniques des patients de manière significative, avec une médiane de consommation de 6 contre 26 mg.Cet effet d'épargne morphinique est déjà retrouvé avec la pose de cathéters périnerveux, mais nous avons aussi établi qu'au sein des sous-groupes de patients bénéficiant de cathéters périnerveux, le groupe « résine immédiate » conserve une consommation morphinique supérieure.Abstract: Post operative pain after ankle or rear foot surgery is severe.
