Freiberg’s infraction: A modified closing wedge osteotomy for an undiagnosed case

Abstract: HighlightsThe pathogenesis of Freiberg’s infraction is not fully understood. We present a 31-year old woman with an undiagnosed Freiberg’s disease.Symptoms are not clearly correlated with Freiberg’s infraction in its early stages; this can lead to wrong diagnosis and treatment.Dorsal closing-wedge osteotomy with single screw stabilization can be a valuable tool when conservative treatments fail.

“…We report on the first known case of nonunion in the setting of Freiberg's disease treated successfully with abaloparatide. The current literature is limited to describing case reports and series of Freiberg's disease in teens and young adults ( Palamarchuk and Oehrlein, 2000 ; Lin and Liu, 2013 ; Biz et al, 2017 ; Al-Ashhab et al, 2013 ; Kim et al, 2012 ; Chao et al, 1999 ; Lee et al, 2016 ; Kinnard and Lirette, 1991 ; Pereira et al, 2016 ; Hayashi et al, 2002 ; Kline, 2008 ). Rarely are complications described, like nonunion.…”

“…In patients with Freiberg's, where there is damage to the blood supply around the head of the metatarsal, this likely will not be successful. Biz, in 2017, presented an extraarticular dorsal wedge osteotomy at the metatarsal neck on a 31-year-old female with 8 months of unresolved pain after conservative treatments was diagnosed with Freiberg's which healed uneventfully [13]. Interestingly, in that case, the surgeons state that due to the lack of knowledge and low frequency of the disease, the infraction was not diagnosed.…”
