Frailty Phenotypes, Disability, and Outcomes in Adult Candidates for Lung Transplantation

Abstract: Rationale: Frailty is associated with morbidity and mortality in abdominal organ transplantation but has not been examined in lung transplantation. Objectives: To examine the construct and predictive validity of frailty phenotypes in lung transplant candidates. Methods: In a multicenter prospective cohort, we measured frailty with the Fried Frailty Phenotype (FFP) and Short Physical Performance Battery (SPPB). We evaluated construct validity through comparisons with conceptually related factors. In a nested case… Show more

“…Others have associated frailty with poor exercise capacity and physical disability. 7 38 We extend these findings by demonstrating that frailty relates to reduced physical function (lower limb muscle strength, exercise capacity, physical activity), dyspnoea, dependency, increased anxiety and depression, and worse emotional distress and health status. These deficits have also been linked to sarcopenia, 15 which is considered a component of frailty.…”

“…One notable exception is a report restricted to lung transplant candidates, among whom frailty was associated with increased risk of delisting or death. 7 Identifying frailty earlier in the course of disease is important, as interventions may then be introduced to prevent functional decline, hospital admissions and/or death in those at high risk. Frailty may prove a valuable way of stratifying patients with COPD for future management as it accounts for multiple deficits that influence disease prognosis, for example, muscle weakness or physical inactivity, 8 including deficits not considered by other syndromes or comorbidity indices.…”

“…In a multicenter prospective cohort study, we evaluated the validity of the Fried Frailty Phenotype and Short Physical Performance Battery frailty measures in 395 candidates for lung transplantation with a mix of lung diseases (65). Among the cohort, 28% were frail by Fried Frailty Phenotype and 10% by the Short Physical Performance Battery.…”

“…Increased pre-transplant frailty is a risk factor for worse post lung transplant outcomes (74,75). Critically ill patients on prolonged mechanical ventilation and/ or ECMO support are prone to developing significant neuro-muscular weakness (76,77).…”
