Frailty

Abstract: Frailty is a common condition in older people. It now can be objectively defined by the Fried criteria. When recognized, early intervention should begin with the institution of endurance, resistance, and balance exercises. In men with testosterone deficiency a trial of testosterone replacement should be considered. Vitamin D deficiency needs to be recognized and treated.

“…[1][2][3][4][5] Multiple instruments to operationalize a definition of frailty have been developed and validated. 1 One index, proposed by Fried, defines frailty as the presence of three or more of five criteria: unintentional weight loss, exhaustion, weakness, slow walking speed, and low physical activity.…”

“…4 Using these criteria, estimates of the prevalence of frailty among independently living adults vary from 7 % of persons older than 65 years to 40 % of persons older than 80 years; prevalence estimates are higher among persons with chronic disease. [2][3][4]6 Frailty, using this definition, is associated with increased risk of falls, hospitalization, disability and death. 4 However, diagnosing frailty in the clinical setting can be cumbersome, because most indices of frailty require a combination of anthropometry and physical function testing.…”

“…1 Frail individuals have a higher risk of developing disabilities in basic and instrumental activities of daily living, falling, institutionalization, and death. Although different definitions have been proposed, two themes seem to better depict the frailty concept: i) loss of functional reserve in multiple areas/domains; ii) existence at a level close to or past the threshold for failure, with reduced tolerance to common stressors.…”
