Background: Hip femoral neck fracture (FNF) accounts for 50% of all geriatric patient's hip fractures. Elderly femoral neck fractures are common with a significant health concern. PPF surgical care can be physically demanding due to severe inflammation, dislocation, and intraoperative fractures, with a high risk of complications. There were some prosthetic designs recorded. Aim and Objective: The aim of study was to evaluate the clinical and radiological outcome of intracapsular fracture neck of femur in elderly treated with cemented bipolar prosthesis Methodology: A prospective study to analyze the outcome of management of intracapsular fracture neck of femur in elderly treated with cemented bipolar prosthesis. Twenty cases of elderly patients with fractured neck of the femur above the age of 56 years with management of intracapsular fracture neck of femur in elderly treated by cemented bipolar prosthesis in the Department of Orthopaedics between September 2018 to April 2020. The short-term functional results were analyzed by using a modified Harris hip scoring system. Results: The patients considered were in the age group of 56 to 78 years with a mean average age of 65.25. The majority of the fractures were sub capital fractures. Our study recorded all the research participants with prosthesis as Cemented bipolar. However, even after using a Cemented bipolar prosthesis, the majority of the study participants (70%) did not experience any complications after the surgery. The medication of choice for femoral neck fractures in patients older than 60 years is cemented bipolar prosthesis. The minimal utility of bipolar hemiarthroplasty has been shown. The cemented bipolar prosthesis should be carried out in a decent setting with a medium to wide trauma or orthopedic center by a professional surgeon. The utilization of an anterior solution to the hip encourages the patient to use it. Conclusion:We conclude that management of intracapsular fracture neck of femur is a good option in elderly treated with cemented bipolar prosthesis. No mortality was observed during the study. The morbidity is not high; the operative procedure is simple, complications are less disabling.

Background:Displaced intraarticular distal humerus fracture has been conventionally treated operatively with various triceps disrupting approaches. These approaches are associated with several complications, such as triceps weakness, nonunion or delayed union of osteotomy, implant prominence, and delayed mobilization of the elbow. We present the functional outcome of intraarticular distal humerus fracture fixation using a triceps-sparing paratricipital approach which allows early elbow mobilization and preserving triceps strength.Materials and Methods:Twenty five patients with intraarticular distal humerus fracture were operated using triceps-sparing paratricipital approach with orthogonal plate construct. There were 16 male and 9 female patients and average age was 42.16 years (range 23-65 years). The mechanism of injury was fall from height (n = 8), road traffic accident (n = 13) and ground level fall (n = 4). Clinical, radiological, and functional assessment with Mayo Elbow Performance Index (MEPI) were obtained at follow up period.Results:All fractures united primarily. At the mean follow up of 13.58 months (range 6-22 months), mean elbow flexion was 121.08° (range 94°–142°) and mean motion arc was 114.92°(range 65°-140°). The mean MEPI score was 94.40 points (range 70–100) with 17 excellent, five good, and three fair results. The mean flexion deformity or extension loss was 6.16° (range 5°–15°).Conclusion:Open reduction and internal fixation of intraarticular distal humerus fractures with triceps-sparing paratricipital approach provide adequate exposure with no adverse effect on triceps muscle strength and allows early initiation of elbow motion. We analyzed, age and injury to surgical interval with relation to functional range of elbow using Z-test which is insignificant.

Results of surgical treatment of distal humerus fractures in adults in Cotonou

Background: Fractures of the distal humerus represent 1.3% of fractures in Cotonou. Their treatment is often surgical. The aim of this work was to assess the results of their osteosynthesis in a disadvantaged environment.Methods: This retrospective study from 2001 to 2015 included 38 operated patients for distal humerus fracture with a minimum follow-up of 12 months. The average age was 33.6 years; the predominance was male (27 cases). The major etiology was traffic accidents (31 cases), According to the AO classification, the fractures were type A (10 cases), type B (02 cases) and type C (26 cases). The average time of surgery was 21 days. The Lecestre plate was the most used (29 cases). An iliac cortico-cancellous graft was used in 03 cases. The mean hospital stay was 20 days.Results: The mean follow-up was 37 months. Functionally, the mean Mayo elbow performance score (MEPS) score was 66.3 points. The results were excellent (08 cases), good (13 cases), average (10 cases) and bad (7 cases). The patients were satisfied or very satisfied in 29 cases. Radiographically, the reduction was anatomical in 34 cases. Anatomical reductions were obtained with Lecestre plates. Consolidation was achieved in 36 cases within an average of 9 weeks. As complications we noted: radial nerve paresis (03 cases); secondary displacement (02 cases); superficial infection of the operating site (02 cases), wire migration or screw retraction (05 cases), non-union (03 cases), malunion in varus (04 cases) and osteitis of the proximal ulna (01 case).Conclusions: Fractures of the distal humerus are infrequent. The anatomical and functional results despite little technical support provided and the long delay before surgery are quite good.
