Background and ObjectiveFractional radiofrequency microneedling (FRM) is widely used as an option for skin rejuvenation, however there is a lack of histological evidence for the various energy delivery systems available. The objective was to assess thermal denaturation of tissue and the wound healing response in monopolar mode versus bipolar mode. Histological analysis was performed to demonstrate the efficacy of automatic impedance feedback system in monopolar mode.Study Design and MethodsIn this study, the acute thermal effects caused by monopolar FRM treatment to the dorsal skin of pigs were assessed histologically by hematoxylin & eosin (H&E) staining. Then, one session of either monopolar or bipolar FRM was used to treat one or the other side of the pig using varying power levels and pulse widths. The acute and chronic tissue reactions were assessed using H&E, immunofluorescence, and western blot analysis at 0, 14, 30, and 90 days after treatment. The efficacy of the impedance feedback system was also monitored histologically.ResultsHigh‐energy FRM treatment produced tissue loss and necrosis. The power level and pulse duration significantly affected the coagulation amount. Histopathology at 0, 14, 30, and 90 days showed that the skin tissue reaction was more pronounced for bipolar compared to monopolar FRM. Immunofluorescence showed the expression of TGF‐β, Ki67, MMP3, and elastin increased dramatically with both modes, but were higher in the bipolar FRM treated side. The automatic impedance feedback system could effectively adjust the output energy.ConclusionsWe found that bipolar FRM produced greater thermal effects, more collagen coagulation, and more pronounced molecular changes compared with monopolar mode in a porcine animal model.

BackgroundThe use of non‐ablative fractionated lasers for skin rejuvenation has been proven to be effective in improving skin texture, and has become popular due to minimal wounding, significantly shorter recovery times and decreased adverse effects.ObjectiveTo retrospectively analyze improvement in skin texture in healthy women aged over 18 years with Fitzpatrick skin types II–IV.MethodsParticipants received three facial and/or neck treatments with the 1570‐nm fractional scanning diode laser at 1‐month intervals. Three months after the last treatment session two blinded evaluators assessed skin improvement using before and after photos. Pain and patient satisfaction were recorded.ResultsSixteen women with a mean age of 45.4 ± 4.1 years (range 35–50 years) and skin type II–IV were included in the analysis. All 16 participants received facial treatments and seven (43.8%) also received neck treatments. Three months follow‐up evaluation showed that the majority of participants had a visible change in the treated areas. Mean pain score was low and decreased with each treatment session.ConclusionSkin treatment using fractional scanning 1570‐nm diode laser improves skin laxity in women with skin types II–IV. Larger studies are warranted to further clarify the efficacy and safety of this modality.

Computer‐assisted full facial imaging systems are currently among the most widely used skin analysis instruments in dermatology and medical cosmetology. These systems offer objective quantitative evaluation of facial skin conditions, and as they are non‐invasive, play an important role in assessing dermatological conditions such as pigmentation, inflammation, vascular diseases, skin texture, the severity of ageing, and therapeutic follow‐up. Although computer‐assisted full facial imaging systems enable quantitative analysis in the scope of medical treatment and cosmetic evaluation, their results may considerably vary because of the influence of environmental and postural factors for improper operation. Furthermore, manual observation is sometimes necessary for experimental work for more accuracy, and familiarity with the imaging principles and application points is necessary to best apply this technique. This report aims to discuss and interpret these systems' imaging mechanisms and explore the primary issues with their application.
