Targeting beta-cell preservation in the management of type 2 diabetes

Type 2 diabetes (T2D) is widely considered a chronic and progressive disease without cure. As beta-cell function progressively declines over time, blood glucose rises. Current management of T2D involves incremental introduction of dietary and drug therapies to achieve normoglycaemia. However, recent studies have demonstrated remission of T2D following bariatric surgery, very low calorie diet or intensive insulin therapy, raising the possibility that the declining beta-cell function in T2D may be arrested or even reversed. The point at which such interventions are introduced in the course of T2D is key for clinical benefit. Future treatment strategies should be revised to target early betacell preservation and thus disease remission. This article reviews the pathogenesis of beta-cell dysfunction and evidence for the clinical benefit of preserving beta-cell function in T2D, and discusses the evidence for beta-cell preservation of current glucose-lowering therapies with particular reference to their effect when initiated at the time of diagnosis of T2D. Резюме. Данная статья является второй частью обзора данных литературы о применении диуретиковВ предыдущей статье говорилось о роли диуре-тиков в лечении артериальной гипертензии (АГ) и предупреждении кардиоваскулярных событий. Рассматривались также вопросы фармакокине-тики и фармакодинамики, классификации и ме-ханизма действия. Данная часть посвящена по-бочным эффектам диуретиков и возможностям их коррекции, правилам выбора диуретиков. Кроме того, сравнивается эффективность различных диу-ретиков в предупреждении кардиоваскулярных со-бытий. Побочные эффекты диуретиковПри использовании диуретиков включаются крат-ко-и длительнодействующие адаптационные системы организма, направленные на сохранение внутрисосу-дистого объема. Краткосрочная переносимость диу-ретиков зависит от реакции на снижение объема цир-кулирующей крови и коррелирует с концентрацией препарата в плазме. Активация ренин-ангиотензин-альдостероновой системы (РААС) и симпатической системы так же, как и подавление секреции натрий-уретического пептида и почечных простагландинов, вносит свой вклад в формирование краткосрочной переносимости. Ограничение потребления соли уси-ливает негативный натриевый баланс и увеличивает терапевтический ответ на тиазиды, в то время как про-должение употребления соли уменьшает эффектив-ность диуретиков. Длительнодействующая адаптация организма на потребление диуретиков происходит на фоне восстановления электролитного баланса. Со-храняющееся обезвоживание запускает длительно-действующую активацию РААС, повышение уровня циркулирующего ангиотензина II, что, в свою очередь, повышает реабсорбцию натрия в проксимальных от-делах и ограничивает поступление натрия в дисталь-ные отделы. Другие объемо-независимые также могут быть вовлечены, включая повышение активности на-триевых транспортеров и структурную гипертрофию дистальных сегментов нефрона. Переносимость ди-уретиков может изменяться при назначении высоких доз или комбинации диуретиков. Например, усиление диуреза встречается, когда тиазиды добавляют к петле-вым диуретикам при выраженных отеках. Скрытая за-держка жидкости может встречаться при резистентной гипертензии, даже несмотря на постоянный прием диуретиков. Повышение дозы диуретиков может улуч-шить контроль артериального давления (АД). Высокие дозы и комбинации диуретиков необходимо использо-вать осторожно, чтобы избежать повреждения почек и электролитных нарушений.Основным аргументом против использования диу-ретиков являются их возможные побочные действия, несмотря на то, что в большинстве своем низкие дозы хорошо переносятся и, как было показано в исследова-нии R.H. The overall impact of glucose lowering on vascular complications and major clinical outcomes, including mortality, in type 2 diabetes is still an open issue. While intensive glucose control has undoubted benefit for microvascular end points, the relationship between glucose-lowering approaches and reduced incidence and/or progression of macrovascular complications is less clear. This review article will discuss the effect of glucose lowering per se as well as the effects of specific glucose-lowering therapies on vascular outcomes in type 2 diabetes. The role of lifestyle changes on cardiovascular outcomes will be also addressed. Recent analyses from large cardiovascular outcome studies (ACCORD, ADVANCE, and VADT) provide new information on factors that modulate the impact of intensive glucose lowering on outcomes, helping to identify the specific clinical characteristics of the patients receiving the intervention that would show a better response. While several studies on cardiovascular outcomes with diabetes drugs are available, they do not clearly highlight a benefit from using a specific medication or will require additional evidence, as for the sodium–glucose cotransporter 2 blockers.
