Follicular Waves in the Human Ovary: A New Physiological Paradigm for Novel Ovarian Stimulation Protocols

Abstract: Ovulation induction (OI) is a cornerstone of human assisted reproduction treatments (ART). Current OI protocols are based on the human follicular dynamics model known as propitious moment theory (PMT), by which follicles continuously grow from the primordial pool without any pattern, and follicular fate depend on the occurrence of a gonadotropin surge. Recently, a new paradigm of human follicular dynamics called follicular waves was revealed using sequential ultrasound examination of 1 interovulatory interval.… Show more

“…It is interesting to note that procedures with proved capacity to increase the superovulatory response of cows (i.e. aspiration of the dominant follicle) (Bungartz & Niemann, 1994) developed more than a decade ago, are just recently being proposed for application in women as a means to increase the efficiency of assisted reproduction (Bianchi et al 2010). Perhaps it is time for human reproductive scientists to pay close attention to reproductive large animal models.…”

“…The FW phenomenon has also recently been observed in women [12,13]. Interestingly, however, evaluation of current OI protocols for assisted reproductive treatment (ART) according to the FW model shows that the start of gonadotropin stimulation is not synchronized with wave emergence [14].…”
