Follicular growth and oocyte maturation in GnRH agonist and antagonist protocols forin vitrofertilisation and embryo transfer

Abstract: Better follicular growth and oocyte maturation are achieved with GnRH agonist treatment. However, both regimens seem to have similar efficacy in terms of implantation and pregnancy rates. Further studies clarifying the effect of the GnRH antagonist on ovarian function are needed, as well as a clear definition of the best period of the follicular phase for the GnRH antagonist administration.

“…This outcome measure was evaluated in 15 studies [8][12], [15][19], [21], [24][26], [28] (2,807 cases; the unified international standard unit pg/ml was adopted, with a conversion factor of 3.67), and heterogeneity was observed among various trials ( P <0.00001, I 2  = 96%). Therefore, a random-effect model was used for the meta-analysis.…”

“…This outcome measure was included in 14 studies [8][13], [18], [22], [23], [26], [28][30] (2,927 cases), and no statistical heterogeneity was observed among various trials ( P  = 0.97, I 2  = 0%). Therefore, a fixed-effect model was used for the meta-analysis.…”

“…The patients underwent COH using the flexible GnRH antagonist protocol, as reported in previous studies (10,11). The injection of rFSH (Gonal F ® ; Merck Serono S.A., Geneva, Switzerland) started on day 2 of the menstrual cycle, and the starting dose of rFSH was 75-300 IU daily and was individualized according to the patient's age, body mass index (BMI), antral follicle count, baseline E 2 , P, FSH and LH concentration.…”

“…Both NOW-PCOS and control-NOW were euthyroid and were undergoing IVF for the first time. All women underwent IVF according to a previously reported protocol [21]. Infertile women with a diagnosis of diabetes mellitus were excluded from the study to rule out potential confounding effects of hyperglycemia on leptin and fertilization rates.…”
