Follicular fluid and supernatant from cultured cumulus-granulosa cells improve in vitro maturation in patients with polycystic ovarian syndrome

Abstract: This adapted IVM system could be used to reach an acceptable result in meiotic competence and competent metaphase II oocytes capable of developing into intact embryos after fertilization and before transfer.

“…Human oocyte quality can be improved with IVM/PMC manipulation of the LH signaling pathway ( Table 2). Human oocyte IVM cultures supplemented with PDE-I [95], AREG [98], CNP [99], and heterologous follicular fluid/CGC [100] improve oocyte maturation and embryo quality. Experimental IVM oocyte maturation rates (range, 70-81.6%) are approaching standard IVF maturation rates.…”

“…This suggests that the normal ''dialogue'' between oocyte and granulosa cells in these early growing follicles is altered. Several studies of messenger RNA (mRNA), miRNA, and long noncoding RNA expression profiles of PCOS have investigated the different human ovarian components, such as granulose cells and follicular fluid (2,10,12,25,26). Also, the involvement of circRNAs in the pathogenesis of conditions resulting in infertility in women has been studied, including in endometriosis (27) and age-related oocyte competence decrease (28).…”

“…Cumulus cells are a subset of granulosa cells that are intimately associated with oocytes. They express functional characteristics distinct from the mural granulosa cells of preovulatory follicles during folliculogenesis and follicle maturation (9,10). Communication between oocytes and cumulus cells is complex and involves paracrine signaling factors.…”

“…Novel rescue IVM approaches are being developed, including the use of heterologous follicular fluid and supernatant of cumulus-granulosa cells in culture medium in order to mimic the intact follicular microenvironment (Madkour et al, 2018). While results appear promising, more data are required to understand the contribution of such approaches to improving clinical outcomes (Escrich et al, 2018).…”
