Follicular dendritic cell sarcoma of the tonsil

Abstract: SUMMARYFollicular dendritic cell sarcoma (FDCS) is an uncommon tumour within the spectrum of histiocytic and dendritic cell neoplasms that can occur at nodal and extra-nodal sites. Besides being rare, these tumours are difficult to diagnose. A 72-year-old man with a painless mass in the right tonsil was admitted to the Mersin University Hospital. Tonsillectomy was performed. Microscopically, the tumour consisted of spindle-shaped cells with large oval to polygonal nuclei. Lymphocytes were scattered among the t… Show more

“…Radiation therapy is generally recommended for extracapsular involvement, high mitosis activity, and abundant nuclear pleomorphism with residual disease [8,10]. Absence of lymphoplasmacytic response, tumor size >6 cm, cellular atypia, or high mitotic index (≥5 per 10 HPF) are considered poor prognostic factors for thyroid FDCS [13,16,17]. Medications such as cyclophosphamide, doxorubicin, vincristine, and prednisone (i.e., lymphoma-based treatment) are among the most commonly used regimens in the treatment of this neoplasm.…”

“…In the study by Hu et al ( 3 ), the misdiagnosis rate was 57%, higher than that in the present study. Carcinoma and FDCS cells are all ovoid cells with avesicular nuclei; however, carcinomas are positive for CK and negative for CD21, CD23 and CD35 ( 21 , 53 ). Similarly to FDCS, GISTs exhibit fascicles, storiform arrays and whorls patterns, and are negative for CK, but immune histochemical markers are positive for Dog-1, CD34 and CD117, and negative for specific markers for FDCS, which may be used to distinguish GIST from FDCS ( 5 , 19 ).…”

“…A MEDLINE and PubMed review of cases of FDCS of the head and neck from 1978 to February 2014 was conducted. Ninety‐five previous cases of head and neck FDCS were identified in the English language literature, constituting a total of 97 cases including our own (Table ) . Information on patient demographics, location, tumor size, adjuvant therapies, recurrence, and mortality was recorded for analysis.…”
