Folinic Acid-Responsive Neonatal Seizures

Abstract: We report three cases of folinic acid-responsive intractable neonatal seizures. All patients were born at term following normal gestation and delivery. In the first infant, seizures began on the 5th day of life and were unresponsive to phenobarbital, pyridoxine, and valproate, but stopped within 24 hours of initiation of folinic acid treatment at the age of 6 months. Her sibling had died at age 6 months with intractable seizures. In the second infant, seizures began in the 2nd hour of life. These were initiall… Show more

“…2,3 In others, the response to folinic acid was variable, including responsiveness to folinic acid alone or to combination therapy. [2][3][4][5][6] Interestingly, in the first report, one individual responded to pyridoxine alone. 2 Another was initially responsive to pyridoxine, but then became resistant and responded to folinic acid.…”

“…1 Folinic acid-responsive seizures have been an enigmatic clinical and biochemical entity of unknown etiology. Patients present with seizures, [2][3][4][5][6] either myoclonic 6 or clonic, 5,6 apnea, 4,5 and irritability 2,4 within 5 days after birth. The electroencephalogram (EEG) shows a discontinuous background pattern with multifocal spikes or sharp waves.…”
