Folic Acid in Obstetric Practice: A Review

Abstract: After completing this CME activity, physicians should be better able to evaluate the need for folic acid supplementation in various patient groups to lower the risk of neural tube defects due to folate deficiency; recommend common, natural and fortified food sources rich in folic acid; and distinguish the effects of folate deficiency in the mother and fetus.

“…Inadequate folate consumption has been associated with cardiovascular and coronary diseases, megaloblastic anemia, stroke, depressive disorder, schizophrenia, Alzheimer's disease, and-despite inconclusiveness regarding the direction of the effect-cancer. 67,69,70 Furthermore, periconceptional folate deficiency increases the onset of various pregnancy-related diseases and outcomes, 71 with most evidence for NTDs. 72,73 In 2007, rice was for the first time successfully biofortified with folate using transgenic breeding techniques.…”

“…Adequate folate intake before and during the first trimester of pregnancy is important for the closure of the neural tube in the fetus, which takes place between the second and the third week of pregnancy. 1 Risk of NTD is inversely associated with folate intake 2,3 and erythrocyte folate concentration. 4 The recommended folate intake during pregnancy is twice as high as the recommended intake for the general population.…”

“…This enzyme has a fundamental role in the transfer of a methyl group to homocysteine in order to make up methionine. The variant of 5, 10-methylenetetrahydrofolate reductase enzyme exists in about 5% to 15% of the general population and it can compromise tissue folate levels [22].…”

“…The active form of folate in the human body, tetrahydrofolic acid, participates as a coenzyme in several essential metabolic reactions. This coenzyme also acts as a vital component in the generation of blood cells and the synthesis and repair of genetic materials that contain nucleic acids [1][2][3][4][5]. Leafy green vegetables, spinach, fruits, and organ foods such as liver are sources rich in folate.…”
