Focal intrahepatic extramedullary hematopoiesis: color Doppler US and CT findings

Abstract: Focal intrahepatic extramedullary hematopoiesis is an extremely unusual condition that can mimic any other solid mass of the liver. Only seven cases have been reported in the literature. Color Doppler ultrasonographic findings of this rare entity have not been described. We report a case of focal mass in a myeloid metaplastic patient diagnosed as focal extramedullary hematopoiesis. Color Doppler findings of this rare condition are described and compared with computed tomography features.

“…The liver is fixed posteriorly and superiorly by the coronary ligament, hypoattenuating at unenhanced CT with a welldefined edge and does not enhance after the intravenous administration of contrast material (Fig 7). These characteristics could be related to a high grade of intralesional fibrosis (14). Extramedullary hematopoiesis may involve the subcapsular region of the liver based on the progression of disease, similar to involvement of the subcapsular region of the liver by lymphoma.…”

“…Iron overload is frequent in congenital haemolytic anaemias and is usually related to chronic transfusion [4]. Although unusual, focal intra-hepatic EMH in this setting can mimic other solid lesions of the liver leading to a troublesome differential diagnosis with hepatocellular carcinoma (HCC) [5, 6]. To our knowledge the specific MR features of intra-hepatic EMH have been reported in just four separated case reports [1, 5, 7, 8].…”

“…Extramedullary hematopoiesis (EMH) is a compensatory mechanism by which blood cells are produced outside the bone marrow when marrow production is unable to maintain the needs of the organism (1). It can be seen in a variety of hematological disorders that lack cell formation such as myeloproliferative diseases.…”

“…It can be seen in a variety of hematological disorders that lack cell formation such as myeloproliferative diseases. EMH usually shows microscopic involvement and its most common sites are the liver, spleen, and lymph nodes (1, 2). In rare cases, such as presented in this report, the involvement can manifest as a mass-like lesion; this uncommon manifestation made it difficult for us to differentiate this disease entity from other focal hepatic neoplasms.…”
