FNA biopsy of secondary nonlymphomatous malignancies in salivary glands: A multi‐institutional study of 184 cases

Abstract: SMSGs originate predominately from the head and neck and are more common in older men. Overall, the FNAB diagnosis of SMSG is accurate, but diagnostic challenges can be encountered, especially in SCC types of SMSG. Ancillary studies are needed for the definitive diagnosis of challenging cases. Cancer Cytopathol 2017;125:91-103. © 2016 American Cancer Society.

“…Lesions range from well‐ to poorly‐differentiated. Roughly 1 in 6 cases of metastatic SCC will demonstrate significant keratinization, a finding rare in other lesions on the differential . More high grade lesions will demonstrate greater atypia, and 5% of lesions show necrosis …”

“…Roughly 5 to 10% of tumors of the major salivary glands represent secondary malignancies, with squamous‐cell carcinoma (SCC) being one of the more common entities to involve them, either through metastasis or contiguous extension from a mucosal lesion . The parotid gland represents the most common salivary gland involved due to the presence of numerous intraparotid lymph nodes.…”

“…However, the presence of the triad of epidermoid, mucinous and intermediate cells in a background of mucin favors MEC over other neoplasms of the salivary gland with oncocytic features. Metastatic breast carcinoma to the salivary gland has been reported in literature . Metastatic breast carcinoma with, apocrine features in particular (Figure A‐F), can be exceptionally challenging to differentiate from intra‐ductal carcinoma, since both the mammary and salivary duct neoplasms with apocrine features share striking morphological similarities and immunohistochemical staining patterns.…”

“…It makes sense that a reliable tool in the evaluation of lymph nodes is represented by ultrasound combined with fine‐needle aspiration cytology (FNAC). This technique has demonstrated interesting results in terms of diagnostic accuracy in head and neck cancer, thyroid neoplasms, and breast, lung, and cervical carcinoma …”
