Fluorodeoxyglucose imaging in healthy subjects with HIV infection: impact of disease stage and therapy on pattern of nodal activation

Abstract: Abnormal FDG accumulation occurs in nodes of subjects with detectable viral loads. Interruption of effective ART results in activation of previously quiescent nodal areas.

“…This could reflect massive stimulation of B cells in the spleen by nonreplicating antigenic material. 34,35 More recently, Lucignani et al 36 assessed whether HIVinfected patients can be differentiated on the basis of the detection of specific locations of viral replication, even in the presence of an apparently optimal immunovirological response to HAART and whether FDG-PET findings can be related to immunovirological variables and AIDS history status. Patients under study were divided into 5 groups: subgroup A1 (full responders, n ¼ 8): current HAART treatment, CD4 þ T lymphocytes 4500 mL…”

“…This is particularly important because HIV-associated nodal reactive hyperplasia and infections may confound FDG-PET interpretations. 27,28 To assess the value of FDG-PET, they were blindly interpreted with the use of commonly accepted criteria. 19 FDG-PET obtained after 2 cycles and at the end of therapy showed high negative predictive values of 91% and 87%, respectively, but poor positive predictive values of 15% and 7%, respectively.…”

“…18 F-FDG has also been used to nonspecifically image sites of viral infection or inflammation, because its detection can be proportional to the glycolytic activity of the cells that trap it (24)(25)(26). In this preclinical study, we demonstrate the efficacy of 18 F-FDG-based PET combined with CT imaging ( 18 F-FDG-PET/CT) as a noninvasive imaging strategy for the sensitive detection of inflammatory disease foci in dengue-infected mice and for monitoring disease amelioration in response to treatment.…”
