Fluindione and drug reaction with eosinophilia and systemic symptoms: an unrecognised adverse effect?

Abstract: Fluindione is not known to be a frequent cause of DRESS. However, the number of reports found is probably underestimated. The seriousness of DRESS, as all immuno-allergic adverse effects, contraindicates fluindione reintroduction. Coumarinic derivatives are the alternatives in patients who need oral anticoagulant treatment.

“…Many studies have reported positive PTs with carbamazepine at 10% or 30%, including 11 cases among 13 patients with DRESS possibly due to this anticonvulsant in the present study, seven out of 10 suspected cases in the study of Lin et al, 17 and 13 ⁄18 cases (at 20% in petrolatum) reported by Santiago et al 3 There are also some reports of positive drug PTs in patients with DRESS 2 in response to abacavir, 18 antituberculous agents, 19 aspirin, 20 carbamazepine, 17,21 cyclins, beta-lactam antibiotics, 9,22 celecoxib, 19 esomeprazole, 22 fluindione, 23 lamotrigine, 2 phenytoin, 3 propylthiouracil, 24 spironolactone and topiramate. 3 In DRESS, herpes family virus reactivation has been reported in 76% of patients, and the main response may be related to an expansion of CD8+ T cells directed against EBV or HHV-6.…”

“…[ 5 ] Association of DRESS syndrome to fluindione was first desbribed by Sparsa et al [ 6 ] Since then, 36 cases of DRESS syndrome associated to fluindione were reported in a 10 years period. [ 7 ] The mean time-to-onset of DRESS syndrome after fluindione treatment was 28.4 days; occurrence was more rapid in patients with previous hypersensitivity syndrome with a chemically related drug. Kidneys and liver were the most frequently involved organs, but no cases of cardiac involvement were observed.…”

“…However, this activity can also be used as treatment for vitamin K antagonists (VKAs), as observed for phenindione derivatives (see Scheme 179) [449]. However, pheindione is not so much used anymore and was replaced by its homologue, fluindione, or other coumarins VKA due to side effects such as hypersensitivity identified for pheindione [450]. However, despite a proven efficiency as vitamin K epoxide reductase, fluindione 609b is used almost only in France because of a lack of study data on old people [451].…”
