Flexor Tendon Graft Attachment: A Review of Methods and A Newly Modified Tendon Graft Attachment

Abstract: A review of the described methods of attachment of flexor tendon grafts to the distal phalanx is presented. The authors advocate the previously described method of attachment consisting of passage of the tendon graft through the pulp with anchorage to the nail. A new modification of this technique is presented, facilitating accurate dissection and placement of the graft with minimal trauma.

“…Thus, we recommend bearing in mind that excessive dissection and formation of transosseous tunnels around growth plate may result in growth disturbances. Distal fixation of the graft has been performed through a number of techniques over the years (Wilson and Sammut, 2003), although the use of bone anchors for distal attachment has been recently preferred (Brustein et al, 2001;Latendresse et al, 2005;McCallister et al, 2006). We opted for the button-on-the-nail technique, but acknowledge risk of nail deformity, looseness, and infection (Kang et al, 2008).…”

“…However, the use of suture anchors is becoming increasingly widespread in hand surgery (Abboud et al, 2002;Brustein et al, 2001;Buch et al, 1995;Khandwala et al, 2004;Silva et al, 1998a, b;Wilson and Sammut, 2003). A variety of suture anchors exists and a number have been tested in the laboratory (Barber et al, 1993(Barber et al, , 2003Barber and Herbert, 1999;Carpenter et al, 1993).…”

“…In 1996, Sood and Elliot reported a technique to attach the flexor tendon to the distal phalanx without the use of a button by passing the sutures through the bone (Sood and Elliot, 1996). Other methods of distal fixation have been reviewed by Wilson and Sammut and include tendon to tendon suture, tendon to bone attachment and tendon through the finger pulp (Wilson and Sammut, 2003). The use of small suture anchors provides a satisfactory alternative and is becoming more widespread.…”

“…There are many methods described for the distal attachment of the tendon. These can be classified as tendon-to-tendon, tendon-to-bone, tendon-throughbone and attachment-around-bone techniques (Wilson and Sammut, 2003) (Figure 4). As with the proximal repair, the same biological principles apply, i.e.…”
