“…The American Society of Anesthesiologists score was highest in the Z-POEM group, followed by the flexible then rigid endoscopic septotomy groups (2.45 [SD 0.69], 2.32 [SD 0.71], and 2.15 [SD 0.69], respectively; P = 0.05). The mean size of the diverticulum was smallest in the flexible endoscopic septotomy group, followed by Z-POEM and rigid endoscopic septotomy groups (28.65 [SD 12.33] mm, 34.85 [SD 14.68], and 36.47 [SD 16.73] mm, respectively; P = 0.003).…”

“…This is a minimally invasive approach with a high success rate (91%) [ 3 ]. However, the rates of symptom recurrence and adverse events are quite high (11.3% and 11.0%, respectively) because the bottom of the diverticulum cannot be clearly identified, leading to incomplete division of the septal muscle [ [3] , [4] , [5] ]. In contrast, Z-POEM allows for clearer identification of the septal muscle, leading to complete septal division.…”

“…The patients were between 42 and 86 years old; 55% of them were men. Complications were minimal and during the procedure, the mean time was relatively short and recurrence only occurred in 5 patients, requiring a second session of endoscopic treatment to achieve a complete myotomy [16,17].…”
