Flexible bronchoscopy contribution in the approach of diagnosis and treatment of children’s respiratory diseases: the experience of a unique pediatric unit in Tunisia

Abstract: Objective: Our study aimed at assessing the role of flexible bronchoscopy (FB) in improving diagnosis and management of children's respiratory conditions in the pediatric unit of FB, newly created and unique in Tunisia. Methods: Retrospective study including all the FB achieved in our pediatric unit from 2009 to 2014. Results: We performed 365 FB in 333 patients aged 46 months on average (1 month -15 years), often under conscious anesthesia (81.6%). FB was performed for diagnostic purposes in 341 cases and for… Show more

“…Currently, no specific therapy has demonstrated efficacy in the treatment of adenovirus pneumonia [ 17 ]. FOB with BAL has now become an important diagnostic and therapeutic procedure for respiratory diseases, including adenovirus pneumonia [ 18 ]. There is paucity of data on BAL for the treatment of adenovirus pneumonia in children.…”

“…Bronchial foreign body aspiration often occurs in children younger than 3 years of age and is more frequent in boys than girls. [ 16 , 17 ] However, aspiration of beads is more common in girls, likely because girls use beads more often as toys. A foreign body is more commonly located in the right bronchus, which may be related to the anatomic features of the right main bronchus: the angle in the right bronchus is smaller while the diameter of the right bronchus is larger than that of the left bronchus.…”

“…The gold standard for diagnosing and managing FB aspiration is rigid endoscopy under general anesthesia . In doubtful cases, flexible bronchoscopy can be used to exclude the presence of FB in the distal airways . A tracheotomy or open chest surgery may be required when conventional bronchoscopy fails to extract the FB or in resource‐limited settings where rigid bronchoscopy is not available.…”

In this case, a long sharp chain was aspirated by a 1-yearold child and removed by bronchoscopy and tracheotomy jointly under general anesthesia.…”
