“…[6] The pathogenesis of bleomycin dermatitis remains speculative and various theories such as micro-trauma, inflammatory oncotaxis, increased melanogenesis, heat-recall and reduced epidermal turnover allowing prolonged melanocyte-keratinocyte contact have been proposed. [4,7,8] There is no specific treatment, but avoidance of bleomycin leads to eventual clearance over months to years. However, re-exposure may produce an aggravated reaction.…”

“…Differential diagnosis of flagellate erythema includes bleomycin, dermatomyositis, and shiitake mushroom dermatitis. 9 This nonclassic variant also has unique histopathologic features including dyskeratosis in the upper layers of the epidermis extending into the stratum corneum without involvement of the basal layer and a superficial dermal infiltrate with neutrophils and lymphocytes. [4][5][6][7][8] While this nontypical variant was first described by Kaur et al 4 in 1994, it may be an underrecognized cutaneous manifestation of AOSD.…”

“…More specific clinical presentations have been less frequently identified such as serpentine supravenous hyperpigmentation (mainly with 5-fluorouracil) [11,12,13,14] or bleomycin-induced flagellate hyperpigmentation [15,16,17,18,19,20,21]. …”
